1. As stipulated in DepEd Order No. 11, s. 2018-Guidelines on Preparation and Checking of

Forms and as per posted guidelines in the Learner Information System (LIS) which directed all public
and private Elementary, Junior and Senior High Schools, State Universities and Colleges (SUCs), Local
Universities and Colleges (LUCs), and Higher Education Institutions (HEIs) offering Kindergarten to
Grade 12 to update the learner’s end of school year status in the LIS and shall comply the following
reports listed below:
