I, FELY AMOR S. MORALIZON, Filipino, of legal age, married to PERCINETTE D.

MORALIZON, with residence address at Prk. Mansanitas, Brgy. West, Tagum City, Davao
del Norte, after having been duly sworn in accordance with law, hereby depose and state:

1. That the copy of my birth certificate issued by the Philippine Statistics Authority, as
well as my Unified Multi-purpose ID (UMID) ID with CRN 0111-5372853, and
Passport ID with No. P497718B issued at DFA Tagum and valid until January 30,
2030, the name indicated is FELY AMOR S. MORALIZON;

2. That, on the other hand, in my Voter’s ID with VIN: 2319-0191A-L1364ASM20000-0,

the name indicated is AMOR S. MORALIZON;

3. That I am known to my community and friends as AMOR S. MORALIZON;

4. That both names pertain to one and the same person;

5. That I am executing this affidavit in order to attest to the truth of the foregoing, and
for whatever legal purposes it may serve.
