MEMORANDUM OF AGREEMENT

KNOW ALL MEN BY THESE PRESENTS:

This Memorandum of Agreement entered into by and among the Bantay Insik Integrated
School represented by its Officer-in-Charge, FLORENCE B. CABUAG, PhD, SHS Asst.
Principal II with office address at Bantay Insik, Sison, Pangasinan and herein referred to as
BIIS;

- and -

The Federated PTA Officers, represented by Ms. SHERYL P. BALTAZAR, in his capacity
as the President of the Association and herein referred to FPTA;

WITNESSETH;

WHEREAS, Bantay Insik Integrated School, being the center of educational activities plays
a lead role in the development of the school children;

WHEREAS, the FPTA as an association of parents, teachers and external stakeholders, the
LGU of Barangay Binmeckeg, the different existing non-government organization who are
committed to be partners of the school in the development of the school children;

WHEREAS, the total development is the responsibility of the school, the parents and the
community;

NOW THEREFORE, for in and in consideration of the foregoing premises, the parties have
agreed as they hereby agree, to sincerely cooperate among themselves in helping in the
development of the school children by performing their roles /commitments as provided
hereunder;

1. Bantay Insik Integrated School

a. Informs the FPTA regarding DepEd plans and programs for the welfare of the
children.
b. Coordinate plans with the FPTA on how these plans and programs can be
efficiently and effectively implemented.
c. Ensure that PTA Funds shall be used only for projects/ programs/ activities
that are agreed upon between the school and the FPTA. Thus, PTA Funds shall
not be used for the school’s utility expenses, office supplies expenses, security
expenses, and other supplies expenses.

2. Federated PTA Officers:

a. Coordinate closely with the school and support all its planned activities.
This Memorandum of Agreement shall take effect immediately upon its signing by the
parties and shall remain in full force and effect until terminated by any of the
contracting parties and due force.

In witness whereof, the parties have hereunto affixed their signatures this 5th day of
April, 2021 at Bantay Insik Integrated School, Sison, Pangasinan.
