    Good day!

    Respectfully submitting herewith the pertinent documents for the initial release of my
    Housing Loan amounting to Php. 1,250,000.00 to wit:

    a. One (1) owner Duplicate Copy of TCT in the name of the borrower with
    mortgage annotation;
    b. One (1) Certified True Copy of the above TCT;
    c. Three (3) copies of Loan Mortgage Agreement duly registered with Registry
    of Deeds. (with original RD Stamp);
    d. One (1) Certified True Copy of the LMA (RD;s Copy);
    e. Three (3) Duly Accomplished / Notarized Promissory Note;
    f. Duly signed Disclosure Statement on Loan Transaction with conformity to the
    borrower;
    g. Notice of Approval with conformity to the borrower;
    h. Interim Promissory Note
    i. Building Plans/ Building/Electrical/ Sanitary Permits duly approved by Building
    Officials.
