Occurrence/ Line report:


T.No.IOTL/TR (Load-1400T), After passing neutral section, VCB not closing with message
"Dis. In Con No.2". MCE OFF/ON then VCB closed but message "Low air pressure-Traction
not allowed", operated A-9 to emergency then put normal, MCE OFF/ON then ok but again
message Traction may reduce if temperature increase", tried to clear section but detained
inside tunnel for pressure dropped, same built and cleared section.
Major/Sec-Kalyan-ChatrapatiShivaji Terminal (KYN-CSTM), Minor/Sec-Diwa-Parsik-New
Mulund Yard (DW-PSK-NGSM), Time-13.15-14.23 hrs.

Log book remarks:


On 24.03.18 while working with T.No.IOTL, between DW-TMBY, after passing N/S
experienced fault message F0301P1, trouble shootedasper TSD & further experienced fault
message F1002P1, trouble shootedasper TSD and finally experienced fault message
F1101P2 & worked upto NGSM and loco made dead.

Shed observations/Investigation details :


Loco arrived to ELS/LGD on 03.04.18 in dead condition. From the DDS it is observed that
the following fault messages logged during occurrence time.

From the above messages, it can be seen that LP initially experienced DJ tripping with fault
message “NSC2:0036-Error operating software”, but SR was not isolated.Instead of
reclosing the DJ, LP wrongly switched OFF/ON the MCE. After switching ON the MCE, LP
experienced a Priority-2 fault message “HBB1:0011-Scav.oil cooler MCB open message
logged”. This message neither caused DJ tripping nor caused isolation of subsystem.
Instead of working the train duly checking the HB-1 panel for any MCB tripping, again LP
wrongly switched OFF/ON the MCE which caused MR pressure dropped below 5.6 Kg/Cm 2
and caused “SR Interlock-main reservoir low” fault message. LP, instead of waiting for
building up the MR pressure, LP again wrongly switched OFF/ON the MCE. Further, LP
getting continuous problem of “NSC2:0036-Error operating software” LP instead of
manually isolating the defective SR-2 and work the train further with single bogie, LP again
switched OFF/ON the MCE. In this process, LP totally 5 times wrongly switched OFF/ON the
MCE and caused huge detention in mid-section/punctuality loss.
Loco was examined at ELS/LGD and found both SRs are working normal. During occurrence
time fault messages 1) NSC2:0036-Error operating software 2) NSC2:0032/33-Error
plausibility Iq2 logged. Hence, NSC-2 card was replaced.
From the records it is observed that the above NSC-2 card of M/s.BT make was
rehabilitated by m/s.Indocen on 05.10.16 and further this card was provided in loco
No.31173 on 21.06.16. The same card failed on 13.04.17.hence, card was sent for warranty
repairs to M/s.Indocen. Further on receipt of the card after warranty attention, the same
card was provided in this loco on 20.10.17.
Regarding, HBB1:0011-Scav. oil cooler MCB open checked and found at times only message
is coming but MCB was not tripped due to MCB aux.I/L defect i.e.at times not closing. This
is an identified problem in Schneider make MCBs. Hence, Scavenger blower MCB auxiliary
inter locks were replaced.

Analysis of root cause/Probable cause of failure:


NSC2:0036-Error operating software fault message logged due to mal-operation of NSC-2
card which was rehabilitated by M/s.Indocen. LP instead of isolating the defective SR-2 and
work further with single bogie(trailing load only 1400tonnes), he repeatedly wrongly
switched OFF/ON the MCE for 5 times which caused huge mid-section detention/punctuality
loss.

1) Sr.DEE/TRO/BB is advised of the crew mis-manipulation and to counsel the crew
suitably to avoid repetition of such incidences in future.
2) M/s.Indocen has been advised of the NSC card failure and to submit the investigation
report.
