II. RATIONALE:

Healthy living is one of our goals in life. We take every opportunity and alternatives that are
available so we can get better as fast as we can whenever we are sick. From herbs to capsules,
from barangay centers up to a real consultation from a professional Doctor. We do everything
to stay healthy by listening to each demands that is showed and advised to us. But what if our
health has been compromised by some matters that are really hard to deal with and are
difficult to solve as well, especially if we are talking about the hospital in general and the need
for money to be able to pay for health expenses. Would it be really a cause of limited healthy
lifestyle just because of the matters that compromise our health?

Even before the pandemic broke out, public Hospital are lacking in every aspects. From the
facility, equipments, medical tools and many more which is why this have raised an idea to the
people that it's better to just keep that certain symptoms that you are feeling to yourself
instead of choosing to go in a private Hospital where everything is quite complete. But of
course another matter is needed in here, the money, if you still want to have a consultation.
But what if there is no money at all for a consultation because you choose to allot the money
for your family's needs? Would it stay just like that? No consultation because you don't have
money?

The 'Barangay Health Center Plus' is proposed to take away your worries of not being able to
consult Doctors just because of having no money at all or because you are not able to consult a
Doctor because the Hospital are lacking in equipments. This proposed program entitled
'Barangay Health Center Plus' have wanted to turn Barangay Center into a clinic and is
proposed because it aims to cater a lot more locals for medical consultation with now added
medical machines that can only be seen in Hospital. Locals or beneficiaries are now welcome to
consult regardless of their age and health problems with just a very low health expenses of
exactly twenty pesos. The goal of this program is not just about giving the locals a low health
expenses but to promote healthy living at a very low cost.

III. OBJECTIVES:

 To address and improve the issues that the Barangay Center staffs are currently facing
so they can come up with a better strategy on how to address and cater the locals
medical needs as early as possible.
 To re-educate people about maintaining health in one's home, this is also to educate
them with the alternatives that they can used as a patient whenever they feel the
symptoms of their health problems.
 To address the overall objectives and plans of this Barangay Health Center plus to help
those who will be covered in this program. As this has been address, the programs will
then ensure an enhancement or improvements to it to help address more health issues.

IV. SCOPE

The scope of this program will be for those:

 Outgoing patients from Hospital who choose to continue their medication at home due
to lack of money (mostly who needed a daily intake of medicines)
 Pregnant individual (single, married and teen Mommies)
 Senior citizen
 Sick adults and children
 Both malnourish children and babies

This is also to help them to lessen the burden of health expenses. And as the candidates have
been lined up to be covered with the proposed program, it aims to cover a minimum of 25-35
and a maximum of 50-100 locals every week regardless of their age and health problems to be
able to recognize and discover the health problems that has not been addressed due to poor
handling on the Health sector in the Barangay or probably due to poor attention span on this
areas.

V. TIMEFRAME

The realistic timeframe for this program as it has been in the movement for debates, sessions,
and implementations would take 9 weeks up to 1 year to be fully be implemented within the
scope of the program, but the process of the program implementation will still depends upon
the availability of the funds from the Barangay and the Municipality. As this has been approved
to be implemented it will last until changes for the program has been suggested and raised.
VI. IMPLEMENTING STAGES

PRE-IMPLEMENTATION ACTIVITIES:

 Sessions and debates over this program implementation.


 Validation and submission of required documentations needed in this programs.
 Approval as signature from the Mayor's Office.
 As this has been fully approved by our Mayor after it has been raised by the Barangay
San Antonio a pre-planning among Technical Working Group from the Municipality must
happen to fully polished the program implementation.
 The Technical Working Group may include:

Municipal

- Municipal Administrator

- Municipal Health Officer (For the seeking and briefing of medical practitioners
who will take part in the proposed program or will be able to provide a suggestion about the
medical practitioners who will take part in the program implementation. They must be
coordinated with the Barangay Health Officer for better discussion.)

- Municipal Nutrition Action Center

- Sangguniang Bayan Chairman on Committee on Health

- Liga President

- Local Finance Committee *Municipal Treasurer, Municipal Accountant, and


Municipal Budget Officer* (Financial statement or budget assessment sessions, once approved,
the proposed budget will be assigned for the purchasing of equipments needed.)

- Civil Society Organization

- Barangay Health Officer

- Sangguniang Barangay Chairman on Committee on Health

- Sangguniang Barangay Chairman on Committee on Appropriation

- Local Finance Committee *Treasurer, Accountant, and Budget Officer*

 Meeting arrangements of the team behind the proposed program for the strategy
making and labor distribution (assistance team and monitoring team including the
Technical Working Group) as well as a short meeting with the medical practitioners.

DURING - IMPLEMENTATION ACTIVITIES:

 Check up will be at 8 o'clock in the morning and ends at 5 o'clock sharp in the afternoon,
for emergency cases that may happen during nighttime the BPSO who will be assigned
at nighttime may refer the patients within the contact Hospital of the Barangay, the
patient may also be refer in the other Barangay who will be having the same program
implementation. This schedule can be adjusted as much as possible depending on the
needs of the locals for health.
 Check ups and consultation are on Mondays until Fridays only, in case of emergency
during Saturdays and Sundays they may be refer within the Hospital contacts of the
Barangay or the other neighbouring Barangays who will be undergoing the same
program implementation. This schedule can still be adjusted as much as possible
depending on the needs of the locals for health.
 Assistance team will be in-charge for the people's organization during the check ups and
consultation in case of delay or interruption. The Assistance Team are:
- Barangay Public Safety Officer (BPSO) (5)
- Sangguniang Barangay Chairman on Committee on Peace and Order.
 Monitoring team will be in-charge to monitor:
1. The issues that has the highest numbers to be consulted within every 7 days.
2. The common reasons of them relying on medical help.
3. Factors that affect their health and making it at risk.

The Monitoring Team are: Medical Practitioners

- Doctor (1)
- Registered Nurse (4)

- Intern (4)

- Barangay Health Worker (4)

 Consultation regardless of age and health problems.

POST - IMPLEMENTATION:

 Analyzation of the program, budget statement review, medical practitioners monthly


salary release.
 Evaluation and enhancement of the program implementation will be discussed with the
medical practitioners and the Technical Working Group to further solidify the program's
objectives or goals.
 Amendments and suggestions for the program will be discussed with the member of the
Technical Working Group to sort out the threats that are being faced by the program
itself.

MONITORING AND EVALUATION:

As the monitoring team, especially the Barangay Health Worker, will be on standby for every
consultation and their aim is to provide a summary of the report after a weekly monitoring.

 Finished documentation with pictures for reports in the Municipality so they can
also be aware of the programs progress
- Annual report for the progress of the program so the members of the
said program would be aware of what is happening before, during and after the
implemention, and how it strive and progress for the rest of the weeks, months,
and even year.
- Financial Statement of the Program.

FUNDING REQUIREMENTS:
The budget allocation for the said proposal will be based from the budget allocation of the first
Barangay who will be having this program, however the allocated budget can be discussed and
can be shared between the Municipality of Los Baños and the Barangay San Antonio. The
budget will depend upon the alloted budget for the Health Department of both the
Municipality and the Barangay.
