Risk Severity” is the outcome/degree if an incident, near miss, or accident H = High Risk did occur and identified as: Catastrophic, Critical, Marginal, or M = Moderate Risk Negligible L= Low Risk Step 2: Identify the RAC (Probability/Severity) rating as E, H, M, or L for each “Hazard” on AHA. Job Steps Hazards Controls RAC The Competent Person shall survey the integrity of the Collapse of Structure structure prior to the start of demolition operations. All workers on site shall wear approved, hi-visibility safety vests, to make them visible to the equipment operators. L DISMANTLING OF TANK Structure Collapse Proceed with demolition in a systematic manner. STRUCTURE Falling and flying Debris Any worker signaling the operator shall always be in plain Pollution sight of the operator. Housekeeping All workers shall remain at least 8-10 feet from the Fire equipment used to perform the demolition. Only workers Slips/Trips/Falls necessary to the operation shall be permitted in the work zone during this operation. Barrels and caution tape will be used to demarcate the L demolition zone. Debris removal will not begin until the removal can be safely performed without exposure to structural collapse or falling debris. Water will be sprayed to keep down the dust from the demolition. Water will not be allowed to accumulate and cause a hazardous condition. Concrete and masonry debris will be removed to a separate container.
