HEALTH AGENT CONTRACT

This contract is entered into by and between ___________________________________ ,

*Monthly Allowance (depends on performance)

First 30 clients - P 3,000
If so; Below 30 clients - P 1,500
Below 15 clients - P 1,000
Below 10 clients - P 500

*Weekly Bonus given every Sunday.

*Additional Bonus Based on Product Retails sold to clients 20%.
*Additional Bonus Based on Packages sold to clients ;
NH Package - P 400
Starter Kit - P 150

Given the below list of duties to do so;

1. ) Given the scripts to Appointment Setting for Wellness Home Services and
Office appearance offer with product recommendations. Requires to close an
appointment per day of at least minimum of 5 clients per household around Cavite
nearby location ( Bacoor, Imus, Kawit, Noveleta, Gen.Tri, Trece Martirez, Tanza,
Naic, Maragondon, Ternate)or Office appointment.
*With home service fee of P250.00 but minimum 5 clients (P100.00 each.)
*Office appointment P100 each clients.
2. ) Must do the calls and set appointment daily and send list all names details of
your clients via messenger with excel template.
3. ) Requires to attend every Sunday here on Head Office for short training and
claiming of weekly commissions.
