B.3 Company Safety Policy

The purpose of this policy is to develop a high standard of safety throughout all
operations of Hor-Ver Dev’t and Construction Corporation.

We believe that each employee has the right to derive personal satisfaction from his/her
job and the prevention of occupational injury or illness is of such consequence to this
belief that it will be given top priority at all times.

It is our intention here at Hor-Ver Dev’t and Construction Corporation to initiate and
maintain complete accident prevention and safety training programs. Each individual
from top management to the working person is responsible for the safety and health of
those persons in their charge and coworkers around them. By accepting mutual
responsibility to operate safely, we will all contribute to the well being of personnel.

Management Commitment

I, GAUVEN A. LADERA, by accomplishing this Construction Safety and Health


Program do hereby commit and bind ourselves to comply with the requirements of the
Department order No. 13, series of 1998-Guidelines Governing Occupational Safety
and Health in the Construction Industry and the applicable provisions of the
Occupational Safety and Health Standards (OSHS).

I will fulfill the requirements contained in this manual and commit to accomplish work
safely, efficiently and with due regard for the environment. Safety considerations shall
be given first priority over all others in matters of design, methods, materials and
equipment.

I will require all employees to act upon the contents of the Safety Manual at all times,
and to be consistently mindful of our ultimate goal in becoming an accident free
company.

B.6 Specific Duties and Responsibilities of the Safety Officer

Duties of Safety Officer

Safety officer assist the project management in keeping the work accident free by
creating and organizing the safety and health program committee. It assures those
guards and personal protective devices are properly used. He assists on job layout and
recommends methods to minimize or eliminate hazards he furnish material to promote
cooperation and create interest or the safety and health program. He also identifies and
provides training to supervision and specialty trade personnel

The safety officer duties and responsibilities include but not limited to.

1. Developing and administrating a project emergency plan.


2. Ensuring that all subcontractors have been screened according to safety criteria and
that they have in place acceptable Safety & Health Program and that they adhere to it.
3. Preparing jobsite safety plans and assuring implementation and enforcement.
4. Arranging for emergency plans and assuring implementations and enforcement.
5. Arranging for emergency procedures and facilities.
6. Performing new hire employees, staff and specialty trade facilities.
7. Performing new hire orientations. This shall be done before starting work on the day
of hire for all employees, staff and specialty trade workers.
8. Orientation and surveillances of subcontractor’s work.
9. Reviewing hazard analysis before any new operation begins. Instructing the
responsible supervisor or foremen on what is required and reviewing it when completed.
10. Performing joint inspections and reviewing inspection reports to others.
11. Notifying appropriate supervisors and foremen of unsafe conditions of deficiencies
and following up to ensure items are promptly corrected.
12. Performing accident investigations for any near miss, property damage or personal
injury. The supervisor and/or foremen shall be part of investigation team.
13. Training supervisor and foremen to conduct weekly toolbox meetings and reviewing
the records for them.
14. Preparing and conducting safety meetings on special topics when situations arise
that necessitate it.
15. Notifying the project manager on all reportable, recordable, property damage and
near misses.
16. Keeping an inventory of necessary safety equipment and supplies including test
equipment.
17. Coordinating necessary training of supervision though the Department of Labor and
Employment or its accredited Training Centers.
18. Issuing stop workers for any operations including subcontractors, that poses an
immediate danger to protect personnel or has potential to cause injury or damage
19. Conducting special training and procedures such as Confined Space Entry Permit
20. Administering the safety the heath plan such as “right-to-know” program for
hazardous substance and maintaining the Material Safety Data Sheets (MSDS).
21. Keeping himself informed and initiate necessary dissemination of all applicable rules
and regulations of the occupational Safety & Health Standards of all the Department
of Labor and Employment.
22. Compelling all required safety reports, accidents reports and any other requirements
dealing with safety or accident reporting.
23. Maintaining employees’ bulletin boards and posters with current safety information
for enhanced communication and safety awareness.
24. Facilitating transport of impaired employees to the medical faculty when treatment
thereof is necessary.

25. Coordinating with proper consultation the return to transitional duty of injured
employees.
26. Designing recommendation and implementing safety promotions programs.

C.7 On-Site Safety and Health Promotion and Continuing Information


Dissemination

To promote safety awareness among workers the following shall be


implemented:
a. Workers Safety Orientation
All workers are required to attend company orientation on the following:
 Company rules and regulations
 Project safety program including all relevant rules and regulations
b. Daily safety talk
Site engineers or project supervisors are required to conduct daily safety talk 15
minutes before start of work covering the following topics:
 Work schedule and commitments for the day
 Present and perceived job hazards and their corresponding control measures
 Housekeeping
c. Safety signage and posters
Appropriate safety signage and posters shall be installed on conspicuous places to
remind workers to be safe always.
d. Weekly inspection
Safety coordinator together with one representative coming from the safety and health
committee shall conduct a safety inspection in the project site areas at least once every
week.
e. Site walk around safety check Site engineer/supervisor together with 1 foreman shall
conduct a daily walk around safety check in their respective worksites.
Safety and Health Information
12.1 Workers should be adequately and suitably:
a) Informed of potential safety and health hazards to which they may be exposed
at their workplace; and
b) Instructed and trained on the measures available for the prevention, control
and protection against those hazards.
12.2 No person shall be deployed in a construction site unless he has undergone a
safety and health awareness seminar conducted by the Occupational Safety and Health
Center (OSHC), BWC and other concerned offices of DOLE or by safety professionals
or safety organizations or other institutions DOLE has accredited or recognized. The
DOLE in collaboration with constructors shall promote programs for the implementation
of these awareness seminars for construction workers.
12.3 Every worker shall receive instruction and training regarding the general safety
and health measures common to construction sites which shall include:
a) Basic rights and duties of workers at the construction site
b) Means of access and egress both during normal work and in emergency situations
c) Measures for good housekeeping
d) Location and proper use of welfare amenities and first-aid facilities
e) Proper care and use of the items or personal protective equipment and protective
clothing provided the workers
f) General measures for personal hygiene and health protection
g) Fire precautions to be taken
h) Action to be taken in case of any emergency
i) Requirements of relevant health and safety rules and regulations.

12.4 The instruction, training and information materials shall be given in a language or
dialect understood by the worker. Written, oral, visual and participative approaches shall
be used to ensure that the worker has assimilated the material.
12.5 Each supervisor or any designated person (e.g. foreman, leadman, gangboss, etc.)
shall conduct daily tool box or similar meetings prior to starting the tasks for the day to
discuss with the workers and anticipate safety and health problems related to 13 every
task and the potential solutions to those problems. The supervisor shall remind the
workers on the necessary safety precautions that need to be undertaken.
12.6 Specialized instruction and training should be given to:
a) Drivers and operators of lifting appliances, transport, earth-moving and materials-
handling equipment and machinery or any equipment of specialized or dangerous
nature;
b) Workers engaged in the erection or dismantling of scaffolds;
c) Workers engaged in excavations at least one meter deep or deep enough to cause
danger, shafts, and earthworks, underground works or tunnels;
d) Workers handling explosives or engaged in blasting operations;
e) Workers engaged in pile-driving;
f) Workers working in compressed air, cofferdams, and caissons;
g) Workers engaged in the erection of prefabricated parts of steel structural frames and
tall chimneys, and in concrete work, form work and other such work;
h) Workers handling hazardous substances and materials;
i) Workers as signalers; and other workers as maybe categorized by TESDA

C.8 Accident and Incident Investigation and Reporting

When an accident or life threatening incident happens on the project site, workers are
obliged to report or inform their supervisor, who in turn will conduct an accident/incident
investigation immediately. The supervisor shall secure the area to prevent any
secondary incident.

Incident report must be submitted to the main office within 24 hours by the site
engineer/ safety coordinator. Attached is the Accident/Incident Report form to be
accomplished by the supervisors.

And in accordance with the Governing Guidelines on OSH in Construction as per D.O.
13, the following shall be implemented:
a) All accidents or incidences shall be investigated and recorded.
b) All work related accidents resulting to disabling injuries and dangerous
occurrences as defined in OSH Standards (Rule 1050) shall be reported by the
Safety Officer. He shall submit a monthly report to the DOLE Region Office,
using standard DOLE forms.
c) Accident report shall contain the minimum information as required in
DOLE/BWC/OHSD/IP-6.
d) He shall notify the appropriate DOLE Regional within 24 hours in case of fatal
accidents.

C.9 Protection of the General Public within the vicinity of the vicinity
of the Construction Site

To protect the general public from the construction hazards, the following shall be
implemented:
 Provisions for the danger signs, barricades, and safety instructions for workers,
employees, public, and visitors such as housekeeping, walkway surfaces;
means of access i.e. stairs, ramps, floor openings, elevated walkways,
runaways and platforms; and light.
 Installation of appropriate warning signs on the perimeter of the project
 Installation of enclosure to prevent unauthorized access or entry to the project
site
 Installation of canopy or covered walkways if danger of falling object is present

C.10 Environmental Control

The provisions of Rule 1070 of the OSH Standards shall be followed if and when
applicable. As a general rule, the following shall be observed:

 Maintenance of adequate ventilation at all times


 Provision of adequate lightings and/or illumination
 Control of dusts and other airborne contaminants
 A daily check shall be conducted by the supervisor to assess the working
environment of the project site

Also, in compliance with relevant environmental laws, rules and regulations set by the
DENR-Environmental Management Bureau (EMB) and by the local authorities, the
following shall be observed:
 Segregation of solid waste to biodegradable and non-biodegradable
 Disposal of waste shall be done in accordance with local ordinances
 Control of dusts by constant watering of dry dust prone areas
 Avoidance on the use of high-intensity high-frequency power tools especially
during night time.

C.11 Guarding of Hazardous Machinery

If hazardous machinery shall be used, the following shall be observed in accordance


with Rule 1200 of the OSH Standards.

A. guard shall be designed, constructed and used that they will:


 Provide positive protection;
 Prevent all access to the danger zone during operations;
 Not interfere unnecessarily or inconvenience operation or production;
 Withstand long use with minimum maintenance;
 Resist normal wear and stock;
 Be durable, fire and erosion resistant;
 Not constitute a hazard by themselves; and
 Give protection against operational contingencies and not merely against
normally expected hazards.
B. Standard guards or enclosures shall be made of materials suitable for the purpose
for which they are designed and constructed.

C. All machinery guards shall be securely fastened to the machine or to the floor, wall or
ceiling and shall be kept in place whenever the machine is in operation.

C.12 Personal Protective Equipment

As a company policy, all workers will be required to have their own hard hat before
hiring. If in case a worker cannot produce hard hat the company will provide free of
charge which should be returned in good condition at the end of the project. Any
damage or lost hard hat shall be charged to the worker thru payroll deduction.

The following are the basic PPE required for this project:
 Hard hat
 Safety shoes or any appropriate foot protection. A rubber shoe is allowed.
 Long sleeve shirt with logo of the company.

The following shall be supplied by company free of charge subject to the following
usage:

Hand Gloves
Cotton -one month
Leather (for welding use) -three months

Mask
Dust Mask -weekly or as needed
Chemical mask -as needed
Full-face mask -as needed
Welding mask -as needed

Eye Protection
Goggles (clear) -every three months or as needed

Ear Protection
Ear muffs -as needed
Ear plugs -as needed

Fall Protection
Full body harness -as needed

C.13 Handling of Hazardous Substances

-No hazardous substances shall be used for this project since it is using ready-mixed
concrete supplied by Davao Solidmix Concrete Product.

C.14 General Materials Handling and Storage Procedures

Materials handling shall be done in accordance with Rule 1150 of the OSH Standards.
As general procedure, the following shall be observed:
 All materials shall be properly sorted and stored appropriately.
 All flammable or combustible materials shall be segregated and stored in a
separate bodega.
 Disposal of materials shall be in accordance with prevailing ordinances and
environmental laws.
 Safe use of mechanical materials handling equipment.
 Secured and safe storage facilities.
 Regular housekeeping as necessary so as not to constitute and/or present
hazards.
 Clearly marked clearance limits.
 Proper area guarding of storage facilities

C.15 Workers Skills and Certifications (for critical occupation)


- No critical occupation or highly specialized works were identified for this project.

C.16 Provisions for Transportation Facilities for Workers in case of


Emergency
In case of emergency, an emergency transport shall be provided by the supervisor to
ferry the victim to the nearest hospital. An ambulance will be arranged if no vehicle is
found suitable for the purpose.

C.17 Temporary Fire Protection Facilities and Equipment


 Fire extinguishers will be provided on site. Dry chemicals with A,B,C shall be
used and periodically checked by the safety officer.
 To augment the fire extinguishers, a drum with full of water and a standby pail
shall be provided.
 Fire brigade shall be formed on each project site and a fire marshal shall be
appointed.

A first aid station shall be provided with first aid kits with appropriate medicines shall be
kept at all times. The following shall be provided as part of the first-aid kits in
accordance with table 47 of the OSH standards:

C.19 Workers Welfare Facilities


The following facilities will be provided:
 Mess hall-appropriate eating areas will be designated in project site
 Comfort rooms-urinal and toilet facilities will be provided
 Water drinking stations-potable water shall be provided to all workers at
designated areas on site

C.20 Proposed Hours of Works, and Rest Breaks


Work timing for this project is from 7:30AM to 11:30AM and 1:00PM to 5:00PM.
Provision of 15-minute breaks in morning between 9:30AM to 9:45AM and 3:00PM to
3:15PM in afternoon and Lunch break of one and half hour shall be observed. A
mandatory break of 30 minutes shall be implemented before rendering overtime after
5:00PM.”

C.21 Construction Waste Disposal
 Disposal of construction waste shall be in accordance with local ordinances.
 Segregation and sorting of materials shall be done prior to disposal to identify re-
usable and recyclable items.
 All solid waste shall be segregated to bio-degradable and non-biodegradable
wastes.
 A 15-minute clean up shall be be observed every afternoon to collect and
dispose of any waste materials.
 All recyclable and/or re-usable materials shall be properly stored for future use or
appropriate disposal.
 Toxic materials and/or chemical waste shall be properly disposed of in
accordance with existing safety, health and environmental regulations.

C.22 Testing and Inspection of Construction Heavy Equipment


- No testing and inspection of heavy equipment shall be used for this project.

C.23 Disaster and Emergency Preparedness Contingency Plan


Site engineers or project supervisors are required to ensure that:
 First aid station is available and has an emergency medical kit
 Coordinate with nearest emergency clinic
 Provide medical transport if required

Medical Emergency:
 In case of medical emergency, concerned personnel must be brought to the
nearest emergency clinic.
 The giving of first immediate first aid to the victim is required. First aid kits shall
be provided on project site. Appropriate emergency vehicle will be provided to
transport the victim.
Fire Emergency:
 In case of fire, inform all workers on site by shouting or any other means to their
attention.
 Grab the fire extinguisher and proceed to put out the fire.
 Contact nearest fire station.
 Report immediately the incident to main office.
Evacuation Procedures:
In case of evacuation, the following should be followed:
 Gather all workers and personnel to designated assembly point.
 Check for any missing personnel/workers.
 Provide transport if required and proceed to designated evacuation site (as
advised by local authorities or government personnel).
Police/Fire Emergency Numbers:
In case of emergency, workers are advised to call the nearest Police/Fire Stations.
Contact numbers of police stations shall be posted on project site.

Standards Operating Procedures and Job Hazard Analysis for the
Following Activities and Other Hazardous Work not outlined herein

a. Site clearing
Prior to mobilization, project site shall be checked for any hazardous materials or
situations that will harm the workers or the public. Site shall be cleared from any
hazardous substances, materials or any situations before allowing entry. All
unnecessary items shall be removed and disposed appropriately. Appropriate
project information and warning signs shall be installed along the perimeter of the
project. Enclosures and/or barricades shall be installed to prevent the access of
unauthorized person in the project site.

b. Excavations
Excavation shall be done in accordance with Rule 1413 of the OSH Standards.
All excavated materials shall be place at least 1 meter from the edge of
excavation. All materials and equipment shall also be place at least 1 meter away
from excavation sites. If water is present, appropriate dewatering equipment shall
be used and shall be operated by a competent worker. Excavation sites shall be
inspected by a competent individual every after a heavy rain.

c. Erection and dismantling of scaffolds and other temporary working


platforms
 Erection, use and dismantling of scaffolds shall be done in accordance with
Rule 1414 of the OSH Standards
 Erection, modification and dismantling of scaffolds shall be done under the
presence of a competent person
 Use of wooden scaffold material is allowed for this project
 Scaffold shall be erected in such manner that it can carry at least 4 times its
intended load
 Access ladder or stairway shall be installed
 Scaffolds shall be inspected by the project supervisor every after heavy rains
 Scaffolds shall be protected from moving vehicles by installing appropriate
barricade
 All working platform above 1.8 meters shall be installed with top-rail, mid-rail
and toe-boards
 Installation of debris net will be done to protect other workers or the public as
the situation may require

d. Temporary electrical connections/installations


 All temporary electrical connection must conform with the prevailing
standards and requirements of the Electrical Code of the Philippines
 Installation, modification and maintenance of electrical systems shall be done
only by a competent individual
 No octopus connection shall be allowed in the project site
 A separate breaker shall be installed for each welding equipment
 A temporary/construction connection shall be applied with the electric utility
company to serve the electrical requirement of the project site

e. Use of scaffolds and other temporary working platforms
When working on elevated platform or on heights, workers are required to do the
following:
 Check structure stability if capable of carrying 4 times its intended load
 Check if access to elevated work area is adequate and safe
 Check if elevated working platform has fall protection such as guard rails, mid
rails and toe boards, if none, inform supervisor to install fall protection system,
if not possible PFAS is required for all workers
 Only workers who are trained to work at heights are allowed
 All equipment that will be used should be properly checked
 Use of personal fall arrest system (PFAS) is required at heights above 1.8
meters
 All works on heights must be supervised by a competent person at all times

f. Working at unprotected elevated working platforms of surfaces


Elevating work platforms:
 Need to be used on a solid level surface; the surface area needs to be
checked to make sure that there are no penetrations or obstructions that
could cause uncontrolled movement or overturning of the platform
 May be used on other surfaces (when designed as ‘rough terrain’ units in
accordance with the manufacturer’s directions; the surface area needs to be
checked for unacceptable penetrations or obstructions
 Need to be clearly marked with the safe working load limit.
 Operators working in travel towers or boom type elevating work platforms
must wear an anchored safety harness. The harness system used must be
able to arrest a fall before the user hits the ground.
 People operating boom-type elevating work platforms with boom lengths
exceeding 11 metres must have an appropriate license.

g. Use of power tools and equipment


Power tools and Equipment used is under lease contract agreement. All power
tools and equipment used by employees are at their workplace in good condition.
Only appropriate power tools shall be used for the job or task. Workers shall be
trained in the use and maintenance of power tools. Power tools shall be carefully
checked prior to use, any defective tools shall be remove from site to avoid
accident use. All tools and equipment shall be kept and stored properly after use.

h. Gas and electric welding and cutting operations


No gas and electric welding and cutting operations shall be done for this project.

i. Working in confined spaces


A fully or partially enclosed space, that is not designed and constructed for
continuous human occupancy, in which atmospheric hazards may occur because
of its construction, location or contents or because of work that is done in it;
Identification of locations or situations where confined space entry may be
required. And use a system to ensure that all potential hazards have been
identified and addressed and only authorized, trained persons enter a confined
space.

j. Use of internal combustion engines


No internal combustion engines shall be used for this project.

k. Handling hazardous and/or toxic chemical substances
No hazardous and/or toxic chemical substances shall be handled for this project
since it is using ready-mixed concrete supplied by Davao Solidmix Concrete
Product.

l. Use of hand tools


All tools (both, company and employee-owned) used by employees are at their
workplace in good condition. Hand tools such as chisels, punches, which develop
mushroomed heads during use, are reconditioned or replaced as necessary.
Broken or fractured handles on hammers, axes and similar equipment are
replaced promptly. Worn or bent wrenches are replaced regularly. Appropriate
handles are used on files and similar tools. Employees made are aware of the
hazards caused by faulty or improperly used hand tools. Appropriate safety
glasses, face shields are used while using hand tools that might produce flying
materials or be subject to breakage. Jacks are checked periodically to assure
they are in good operating condition. Tool handles are wedged tightly in the head
of all tools. Tool cutting edges are kept sharp so the tool will move smoothly
without binding or skipping. Tools are stored in dry, secure location where they
won't be tampered with. Eye and face protection is used when driving hardened
or tempered spuds or nails.

m. Use of mechanized lifting appliances for movement of materials


No mechanized lifting appliances for movement of materials shall be used for this
project.

n. Use of construction heavy equipment


All equipment (both, company and employee-owned) used by employees are at
their workplace in good condition. Appropriate safety glasses, face shields, and
similar equipment are used while using equipment that might produce flying
materials or be subject to breakage. Heavy equipments are checked periodically
to assure they are in good operating condition.

o. Demolition
The removal wholly or in part, and satisfactory disposal of old pavements, and
other obstruction which are not designated or permitted to remain, except for the
obstructions to be removed and disposed off under items of the contract. It shall
also include the salvaging of designated materials and backfilling the resulting
trenches, holes, and pits. Prior to mobilization, project site shall be checked for
any hazardous materials or situations that will harm the workers or the public.
Site shall be cleared from any hazardous substances, materials or any situations

C.24 Penalties/Sanctions for Violation of the Provision/s of the SH Program

The following shall serve as a general guide in giving penalties and sanctions for
every safety violation by the worker.
UNSAFE ACTS not resulting to harm, damage property and/or loss to production shall
be considered minor violations.
1st Offense - verbal warning
2nd Offense - written warning

3 Offense - final written warning
4th Offense - 3 days suspension

5 Offense - dismissal

UNSAFE ACTS those results to harm to other workers, damage to property and/or loss
to production shall be considered a serious misconduct and shall be meted with the
following sanctions:
1st Offense -1 day suspension and/or payment of the cost of damage/losses
2nd Offense -3 days’ suspension and/or payment of the cost of damage/losses

3 Offense -6 days’ suspension and/or payment of the cost of damage/losses
4th Offense -Dismissal
