 Facilitates referrals/authorization as appropriate within the
benefits structure or through extra-contractual
arrangements.
 As a member of a interdisciplinary care team (ICT)
participates in interdisciplinary conferences in order to
monitor, and evaluate the care management plan and
modify as necessary.
 Follow up with Outreach Teams and Participate with
Health Fair Organization to present the functioning of
Nurse Case Manager and the help of Nurse Case Manger to
members.
 Teaching: Diabetes, HTN, COPD, CHF and how to live
and take care yourself face to chronic illness.

  Assess physical and psychosocial status of patients and
families within 24 hours of admission. Teaching patients,
families about Hospice, comfort care and the goal of
Hospice.
 Provide skilled nursing as determinate by the
interdisciplinary plan or care according to the Palliative
Care Compass Guide. Monitor the
recertification/reauthorization process.
 Coordinate the patient’s and family’s care and services
including those of the team Social Worker, Chaplain,
Volunteer and Physician. Serve as the patient and family
advocate by maintaining regular communication with
attending physicians, long term care facility staff.
