NARRATIVE REPORT OF A COVID-19 PATIENT

I, Elenita B. Prantar Teacher-I of Ipil Heights Elementary School-Ipil District been diagnosed as
COVID-19 patient. Last January 13, 2021, I was not feeling well while sorting the printed modules of
GIYA for distribution. I noticed that I have symptoms of COVID-19 but I didn’t pay attention because I
thought I would recover. I was taking medicines but I still felt the same way from school until I came
home. That’s why I’ve decided not to report further next week so that I cannot spread the bacteria or
maybe virus to them. Since, I still feel the same way , I started home quarantine last January 18, 2021
and decided to do a SWAB Test last January 20,2021 to make sure how I feel specially now during the
pandemic. Two days later, my SWAB Test came out positive for SARS COVID-19. Since that day, I
have been isolated at IPIL LIGTAS COVID CENTER from January 22,2021 to January 31, 2021 and
continued as Home Quarantine up to February 5, 2021. I returned back to work on February 8, 2021 as
I was given a Quarantine Clearance from the Ipil Municipal Health Office signed by Adnilre Drapiza-
Verzon, MD.FPSMS our current Municipal Health Officer.
