“Reporting Emergency, Psychiatric Emergencies Patient”

Good morning Nurse, here I am Nurse Widha will report about patient Y with psychiatric
disorders schizophrenia at risk of suicidal behavior in the emergency room which will be
moved to the samba ward. The patient's identity is as follows: name : Miss Yuli Astuti, age:
23 years old, gender: female, marital status: not married, religion: Islam, education : Senior
High School, occupation: not yet working, address: Ambarwati, Sukarame, Surakarta, Central
Jawa, number registration: 23407. As for the identity of the person in charge among others :
name : Mrs. Ira, age: 55 years old, gender: female, occupation: farmer, relationship with a
patient : patient's mother. From the results of the assessment, it was found that the
following vital signs: blood pressure: 110/70 mmHg, respiratory: 22 x /minute, pulse: 103
x /minute, temperature : 36.0 ℃. The patient's condition was obtained from the assessment of
the patient's mother and observation. The patient's mother said before being taken to the
hospital the patient was angry and wanted to injure herself by locking the door in the room.
The patient slammed the things she saw such as glasses, plates, tables while screaming out
loud. The procedure is performed in the patient's home by attaching a rope to the patient's
hands and feet. The patient's mental status when observed and studied included : the patient's
appearance was untidy, the patient spoke loudly with high tones and the words were not
clear, the patient's eyes were red, the patient's feelings and emotions were unstable, the
patient's motor activity was tense, the interaction during the patient interview not cooperative,
eyes look sunken, eye contact is lacking, the patient often daydreams and only looks at one
point. The patient's mother said the patient had a history of mental disorders since 3 years ago
due to rape and violent behavior by the patient's boyfriend. Patient’s feel unworthy and
useless after the incident of rape. Patient’s often daydream, laugh, cry, and even cut their
hands with sharp objects. There were several injuries on the patient's body including a wound
on the patient's left hand because the patient had attempted suicide by cutting her hand. In the
emergency room, restraints are performed to calm the patient. The patient's left hand is given
wound medication and gauze dressings. The nurse also collaborated with the doctor in
administering the 200 mg sulpiride injection drug. After the patient feels calm and
emotionally stable after being given the action, the patient is immediately transferred to the
samba ward for further treatment. Thank you.
