CONDUCT OF COVID-19 MITIGATION AUDIT

To: Assistant Schools Division Superintendent

Chiefs, CID and SGOD
Education Program Supervisors
Public and Private Schools District Supervisor
Elementary and Secondary School Heads
Teaching and Non-Teaching Personnel
All Others Concerned

1. The Department of Education (DepEd) issued DepEd Order No. 29, s. 2021
which marked the opening of classes for School Year 2021 – 2022. As part of
the preparation of the schools, the School Health and Nutrition Section will be
conducting a COVID-19 Mitigation Audit to the Division Office and schools.

2. The audit aims to monitor the compliance of all schools to the required
minimum health protocols and hazards control to mitigate the effects of
COVID19 to our learners and personnel.

3. The School Health and Nutrition personnel shall schedule audit/visit to all
schools to validate the compliance using the enclosed checklists. The following
shall be the steps in the conduct of said audits:

3.1. School heads shall be notified at least three (3) days ahead of the
scheduled visit.
3.2. An opening meeting following minimum health protocols shall be
done with the Audit Team to be attended to by the following:
a) School Head and/or Representative
b) Covid19 Focal Person
c) Contact-Tracing Focal Person
d) School Nurse
e) DRRM Focal Person
f) Mental Health Focal Person

3.3. After the meeting, Audit Team members together with the School
representative shall divide into groups to assess key areas using
the checklist.

3.4. A closing meeting shall be done to communicate the findings and

allow clarifications on the matter.

3.5. An Audit Report listing the audit findings shall be furnished to the
School Head with a copy submitted to the OSDS for compliance.

3.6. Once Audit Report is received, the School has 1 week to
communicate to Audit Team the compliance to the audit findings
with attached proofs. Depending on the attachments and findings,
subsequent audits may be scheduled.
4. Widest dissemination of and compliance to this Memorandum is expected.
