SUBJECT : Survey for DepEd Personnel Eligible To Receive The Quadrivalent Influenza
Vaccine

1. This is to announce the availability of the Quadrivalent Influenza Vaccine for School Personnel
at the Division Office Clinic on November 26, 2021. In accordance to the DepEd Task Force
COVID-19 Memorandum No. 464 dated 5 August 2021 titled Reiteration of Corresponding Work-
Related Guidelines Per Community Quarantine Classification and Ensuring Observance of Strict
Health Measures in Work Settings in View of the Threat of The Delta Varian Transmission, please
take note of the following mitigation protocols adapted within SDO CSFP.

2. The DTFC continues to underscore the importance of vaccination as an extra layer of protection
against COVID-19, in addition to the required health standards.

3. The addition of Quadrivalent Influenza Vaccination for qualified DepEd Personnel under Project:
Acute Immune Response Welfare and Yonder (Project AIRWAY) is part of the SDO’s initiative to
help boost the immune system and protect against other possible complications to the respiratory
tract that may be brought about by COVID-19, Pneumonia, and Influenza virus infection.

4. In line with this and for proper scheduling, all identified vaccine recipients must accomplish the
Flu Vaccine Survey: SDO CSFP (DO Personnel, School Heads, Teaching and Non- teaching
personnel, Deployed AO2 and ADAS, eco aides/ utility/ security guards) Google Form Link that
may be accessed using the official deped email account via https://bit.ly/FluVaccineSurvey or the
link will be sent via the Official DepEd CSFP SGOD and Public School Heads, School Health
Leaders and Expanded Execom FB Messenger Group Chat.

5. The deadline for the accomplishment of the Survey form is on November 25, 2021 at 12:00PM.

memorandum reiterates the activities and other required updates in the following enclosures:

7. Please consider that the information to be collected shall guide the Department’s leaders across
all governance levels in making appropriate decisions in response to the ongoing pandemic.

8. The list for the schedule of vaccination of qualified personnel will be released in a separate
memorandum.

Enclosure No. 1 – Reminders for Quadrivalent Influenza Vaccine Transport, Storage and
Inoculation

1. The pre-filled syringe of the Quadrivalent Influenza Vaccine for School Personnel will be available
on November 26, 2021 (Friday) at the Division Office Clinic.

2. All Division Office and School Teaching and Non-Teaching Personnel eligible for vaccination will be
scheduled for inoculation by the Division Office Health and Nutrition Personnel except for Pampanga
High School who will be vaccinated by the in-house Medical Personnel of the school.

3. The time of availability for pick up of the allocated vaccines for Pampanga High School will be advised
via Direct Message to the PHS Nurse II. The Nurse II or any authorized school representative must
claim the allocated vaccines on the same day.

4. The vaccines must be transported using a durable and suitable cooler with ice already available upon
pick up. The cold chain of vaccine transportation must be maintained.

5. The allocated vaccines must be stored in a refrigerator with a temperature ranging from 2 – 8o C
(5o C average). Do not store vaccines in the freezer because the quality and components of the vaccine
will be lost once frozen.

6. The division office and Pampanga High School shall store the allocated vaccine and will only be
opened on the day of scheduled inoculation of the eligible personnel by the Health and Nutrition Unit
Personnel.

7. On the day of scheduled inoculation, vaccine recipients must have accomplished the Quadrivalent
Influenza Consent Form. Please take note that the available vaccine provides protection against the
identified strain of influenza virus for year 2021 only, hence, once the flu vaccine for 2022 becomes
available, it would be advisable to receive the latest available vaccine.

8. Recipients who have received 1 dose of flu vaccine within the year 2021 are no longer required to
receive the flu vaccine. Recipients who have received the COVID19 vaccine within 4 weeks will be
deferred and rescheduled to the latest possible date before the expiration of the donated vaccines.
