    Pursuant to the Joint Memorandum Circular No. 2021-001 on Guidelines on the Graudal
    Reopening of Campuses of Higher Education Instutions for Limited Face-to-Face Classes
    during the Covid-19 Pandemic, the undersigned would like to seek your assistance in
    requesting support from respective heads and offices on the following requests/suggestions:

    1. Additional Nursing Arts Laboratories (4 Rooms) to accommodate and anticipate:


    6 RLE Groups AY 2021-22
    10 RLE Groups AY 2022-23
    21 RLE Groups AY 2023-24
    2. Organization Chart of the Crisis Management Committee with photo
    3. Video Presentation (walk through) of the facility (from gate to Nursing Arts Lab and vice
    versa)
    4. Creation of Information, Educational and Communication (IEC) handout materials for
    limited face-to-face (print and/or e-copy)
    5. Consultation with LGU regarding occupancy capacity
    6. Creation of Office of Student, Affairs and Services (OSAS) Continuity Plan ensuring
    continuous provision of student support to be submitted to CHED
    7. A mock accreditation/ compliance of the limited face-to-face application by Mr. Mark
    Angelo R. Calabia, DOH Accreditor – Community Safety Policy
