ACCOMPLISHMENT REPORT
1. Activities Conducted
Pre-implementation Phase
 Checked the learners’ performance in the first and second quarter together with
their returned self-learning modules, answer sheets and performance task.
 Identified the students who need improvements and enhancements, and those
who only need to complete their missed activities and performance tasks.
 Conducted a small group discussion regarding the approach in the delivery
of the bridging activities.
Implementation Phase
 Informed the parents of the students about their performance in the first and
second quarter. The students who got fairly satisfactory grade were given
remedial learning activities to cover least mastered competencies. The students
who got satisfactory grades were given activity sheets and other resources for
them to continue learning during the two-week “bridging the gap”.
 For the students who are residing beside the school premises, their
parents were asked to claim some learning activity sheets and other tasks
given by their subject teacher.
 For the students who are residing in the far-flung areas and/or are not
available of the given schedules, home visitation was being done by their class
advisers.
 Conducted a phone conference with the parents to monitor the
performance of the learners.
 Provided feedback regarding with their assessment results and grades to
give guidance for improvements for the low performing students.
Post-implementation Phase
 Collected and checked all the learners’ output.
 Checked through phone consultation if the revised and modified activities
were effective towards the improvement of the improvement of the learner’s
performance and engagement.
 Observed and examined learner’s enhancements with their learning after the
implementation of the program.
 Report to the grade level coordinator regarding the performance of the
students.

2. Impact of the Bridging Activity to the Learners

The teleconference began exactly at 7:30pm via google meet. The meeting was headed
by Mrs. Donna Mia O. Plantado, Grade 9 Level Chairperson. She mentioned that the meeting
was about the enrollment data, parent’s orientation, classroom preparation considering the
IATF guidelines or sanitation, supplies, schedule of learning modules distribution/retrieval and
other matters.
The first topic was about the enrollment data. She mentioned that the 8 sections have a
total of 51 number students except 9- Topaz with 50 students. She asked Ms. May Valiente
about the Learner’s Enrollment Survey Form of the new enrollees. Ms. Valiente mentioned that
the adviser could get the hard-copy of the forms to the registrar office and student’s former
adviser.
She also reminds everyone by the schedule of the upcoming Parent’s Orientation. The
said assembly was scheduled on August 14, 2020 (Friday). There will be a morning and
afternoon schedule. Morning Schedule are the following sections: Pearl, Emerald, Sapphire,
Region V
Amber & Diamond while the afternoon schedule; Turquoise, Obsidian, Topaz, and Amethyst. In
addition, it was also stated that the orientation will be divided into 3 batches with maximum of
15 attendees every classroom. Teacher’s attire will be the new polo shirt wearing school ID. The
adviser should provide 2 copies of attendance sheet. She highlighted to provide
separate/additional form. The first column is the name of the students. Second column are the
name of parents/guardian who will receive the modules and last column is their active mobile
number preferably SMART network. She also suggested to use PowerPoint presentation,
recorded audio/video or manila paper to be posted inside the classroom to easily discuss the
agenda. Minutes of meeting and pictures of the participants should be provided by the adviser.
Furthermore, Mrs. Plantado discusses the IATF guidelines or sanitation. She suggested
that we should use classroom with 2 doors for the entrance and exit of the attendees. We are
also required to post signages stating the IATF health protocols outside of our classroom. She
also mentioned that the parent/guardian coming from the barangay who has covid cases and
with 37.4 body temperature are not allowed to enter inside the campus.
In addition, for modules distribution, it was announced to be scheduled every Monday

while the retrieval will be every Friday. She also required all the subject teachers to provide
their own boxes with plastic cover and label of their subject areas. She recommended to
download our modules for us to review and study the SLM.
Other matters were also discussed. She mentioned that our payments for ACCSSA
tickets are for refund. She reminds us about the solicitation for stakeholders. She emphasizes
that there will be no personal solicitation instead she suggested to do it in proper way by
getting a solicitation letter from the brigada coordinator. Clarification and verification on the
subject matter were encourage during the meeting. The teleconference ended at exactly 8:15 in
the evening.
