Conduct Cards

Huntsville Intermediate School

Conduct Card
Top part of conduct card is all about the student. Teacher will fill out student information on each card issued.

Infractions
Infractions will be given throughout the day in all classes. Teachers will mark the infraction using the # codes at the bottom of card. The teacher who marks the last infraction will issue new card, contact parent, & document. 5 infractions will fill one card.

Consequences
1st card - Parent Contact
Last teacher to fill card will contact parent.

2nd card - Conduct Lunch


Student will receive 1 lunch detention Parent will be contacted

3rd card - Conduct Detention


Student will be assigned 1 hour of after school detention Parent will be contacted via phone & sign detention form.

4th card - Office Referral


Official referral will be written up Parent will be contacted by office Consequence will be administered by administration.

Parent Signatures
Parent Signatures will be checked each Wednesday during ZAP. Students need to show card to parents and they will sign for the previous week before Zap on Wednesday. An unsigned card will result in a mark.

Rewards
3 positive rewards for different levels! Blank or ZERO marks - Lunch in Atrium 4 marks or less - Just Dance party 8 marks or less - Hat Day Zero marks receives all 3 rewards! 4 marks or less = Just Dance & Hat Day

Restroom Passes
Students have 20 Restroom passes for each 6 weeks. Teacher will sign off each RR pass throughout the day. RR passes will be documented on each new card.

Requirements
Conduct Card must be on student at all times in the classroom. A lost, defaced, or stolen card = to a filled card All teachers and staff will give marks based on infractions & not warnings. Infractions will be documented and used by teachers and administration. These cards are for classroom infractions. Severe infractions will still be automatic write ups.

Goal
Make our school an engaging learning environment for all students to feel safe and to succeed.
