Temporary Certificate of Enrollment

TO WHOM IT MAY CONCERN:

THIS IS TO CERTIFY that REZY P. MABAO, a BSA-II student in PHINMA-CAGAYAN

DE ORO COLLEGE is a bona fide City Scholar whose scholarship renewal is on hold due to
delayed grade submission and or pending deliberation of his/her scholarship status.

This is issued upon the request of the above-named student that he/she is allowed to
enroll in your school even without or minimal down payment. However, the City Government
through the City Scholarship Office shall provide official Master list of Scholars and Certificate of
Enrolment once student is cleared or renewed.

This certification is for enrolment purposes only. Should the student fail to renew
his/her scholarship, dropped in the middle of the semester and or is removed from the program,
the student shall take responsibility of incurred tuition and other fees of the currently enrolled
semester.
