BROKEN RIGHT ARM DUE TO FALL INCIDENT

You are a member of the safety committee and have been notified to report to
the maintenance department to conduct an investigation of an accident. John
Doe, a carpenter, has been taken to the hospital with bruises on his right hip
and fracture of his right forearm. You and your group must conduct an accident
investigation and write a report to determine the root cause/s and
recommended corrective actions.

ACCIDENT CASE SCENARIO


 John Doe, 45 years old, has been a loyal employee of ACME
Construction for the last 20 years.
 Due to the delay in the project, he was tasked to work seven 12-
hour shifts in a row to finish the portion of the ceiling.
 It was his 7th day to work on a 12-hour shift when the accident
happened.
 Accident happened at near the end of shift (4:40pm)
 Witnesses say that Mr. Doe was standing on the top step/rung of
the ladder prior to the incident.
 The employee was approximately 10feet above the floor.
 The position of the base of the ladder was too near the wall
because of the scattered debris at the floor.
 No fall arrest or restraint system was used.
 A ladder inspection policy is in place, but there is no evidence that
the ladder has ever been inspected. The policy holds that
scaffoldings and ladders must be inspected before its usage.
 On the incident scene, shattered wood debris from the ladder was
noted.
 During interview, the facility manager said that he was about to
inspect the ladder during the time John Doe uses it.
