Work related

Brief description:

Mr. Asiras spotter of RBI. While transferring sheetpile using vibro hammer it
slipped through the left calf leg of Mr. Asiras that caused injury. The sheetpile
located at uneven surface and directly sloped to the position (abutment area) of
Mr. Asiras. The other sheetpile was not properly position and across the
orientation of stock pilling.

Action taken

Action taken:
1. Bring to the nearest hospital together with CMHS team.
2. Initial investigation was conducted to gather more information.
3. Temporary work stoppage at abutment area.

1. Based on his pre – employment medical examination dated Feb. 7, 2019,


patient is Class B: fit to work with minor ailment / defect (dental caries).
2. Access stairs where the worker passed were clear from any obstruction, also
the stairs is properly illuminated.
3. The worker worked at 22nd residential (unit 2203), he used the stairs and not
the CMAX due to the CMAX was not available on that time. He went down
from 22nd residential to Ground Floor using the stairs.
4. Based on the Overtime Request of the worker, it was indicated that the OT
that will be rendered by the worker is until 12mn.
5. The worker was alone during the incident happen. But the time he was going
down stair he was with his co worker then separate at podium 1 because his
co worker will get his motorcycle at the parking area (podium 1).
6. He ate during project overtime (6pm-7pm)
7. Based on the inspection, the worker was wearing complete PPE. Safety
shoes is in good condition on the time of the incident.
8. The patient was seen approximately 15 meters away from the stairs where
the incident happens.

9. Based on the statement of their supervisor (leadman) he had 5 workers


rendered overtime but in different areas. Mr. Mendoza and Mr. Crispo were on
the same floor and unit (2203).
10. The CMAX schedule was just until 10pm.

Statement of the witness

Mr. Crispo stated that he and Mr.


Mendoza were together when they
go down to the stairs from 22nd
residential up to podium 1, but then
they were separated on that floor
due to Mr. Crispo will go to parking
area to get his motorcycle. Upon
crossing the Ground floor using the
ramp he saw Mr. Mendoza sitting on
the ground and asking for help. He
immediately went to the office of EHS
to seek for assistance for his co
worker.
Statement of the witness

Mr. Bidllam stated that when he was


about to log out at their Biometrics
(located at Asiapro Office, near EHS
Office) he saw Mr. Crispo, and Mr.
Crispo told him that one of their co
worker had an accident (referring to
Mr. Mendoza) so he accompanied Mr.
Crispo to help.
Root cause & Contributing factors

Root cause:
1. Improper stock pilling of sheetpile.

Conclusion:
• The worker misstep on the stairs that caused on his ankle to twisted.
