Scenario:
Level 1 exercise

Objectives:
 To practice PM in incident command (On Scene Commander)
 To practice HSE Team in his role of muster coordinator
 To practice all other staff in Emergency procedures mustering
 Simulation of fire intervention check of procedure compliance
 Raise awareness of the danger: its consequences and how to mitigate through findings and investigations

09:20 During commissioning activities/test, an electrician was working at the 2nd floor of the
substation. Due to a plugging operation fault of the equipment switchgears SW, an
electrical fault occurred.
09:24 A fire broke out causing panic and fear among the electricians working in the area
09:25 Calls were shouted loudly that a fire accident was about to occur in the work area
(SW) room.
The Supervisor immediately inform HSE Dept. And Site Supervisor in order to activate
09:25 general alarm to inform all people at site about the event ongoing.
09:25 HSE SPV activated immediately HSE manager in order to activate ERP immediately for
the first response.
09:25 HSE SPV immediately inform that a worker was falls and invited security to activate
siren.
09:26 HSE Supervisor immediately called HSE Mgr. which immediately called PARAMEDIC to
rescue IP. ERT team was immediately sent to incident area. Site Manager, once
informed, informed ENI Mgr. as per ERP flow chart.
09:26 PM ABG inform by Mobile Phone ENI PM. ENI Pm activate immediately COMPANY
communication process as per ENI ERP.
09:27 Fire Watcher activated evacuation and extinguished the fire with a fire extinguisher
09:28 ABG HSE manager requested the security SPV , to closed main gate immediately
09:29 Siren have been activated and HSE Mgr. all workers to move to Muster point near the
offices area of ZMDEI/substation Project.
09:30 HSE supervisor made the head count at muster point in cooperation with security team
using the site access register.
09:32 HSE personnel briefed employees on the scenario and instructed workers on fainting,
how to handle these situations, and how to respond during a real accident.

09:42 After finish the TBT. ABG PM was informing on phone call to ENI PM . “all clear”
signal, the worker returned to the normal activity.
09:45 PM ENI Mgr. have been informed about closeout of the drill

During the debriefing the further improvement steps were discussed.

Observations
Positive:
 All ABG staff followed the evacuation procedure and know exactly where they have to go in
case of emergency, the personnel spent 3 minutes to arrive at muster point. (after alarming)
 Nurse response was prompt.
 All activities have been stopped by HSE SPV ABG. All PTW have been temporarily suspended
and withdrawn as per ER protocol.
 Dealing with the operation and the watcher performance was very excellent (sufficient
experience with integrated tools)
 The emergency team was a good performer and followed all the requirements to deal with the
PI which was transferred with all professionalism with the use a stretcher
 Active and positive cooperation with Security ABG
 HSE team swapped the work area to ensure that the evacuation of the site is completed.
 Coordination between ABG, CPECC to inform them Drill exercise was indicted on 10 Apr 2023
the day before.

For Improvement:

 The security team did not respond to the administration's notifications by opening the
external gates for visitors during the practice DRILL Scenario
 WEIR snubbed the exercise. We need to implement a coordinated ERP between ABG-WEIR-
ZFOD and ENI.

 Windsocks need to be visible everywhere in the area.

Recommendations:

 Communication between security and the HSE department needs to be improved. If the input
and output register is not sufficient, a monitoring system such as that of the T_CARDs must
be implemented. SECURITY must respect the information received during the DRILL.
 Improve radio communication on site.
