QUITCLAIM, RELEASE AND WAIVER

I, Alvin F. Tolentino, Filipino Citizen, of legal age, married and with postal address at 209 Rizal
St. Malapit San Isidro Nueva Ecija , after having been sworn to in accordance with law, do
hereby depose and state that:

I received the amount of THIRTEEN THOUSAND NINE HUNDRED SIXTY TWO PESOS
(Php 13,962) as a form of final compensation for my services with Kenshin Japanese Izakaya
Restaurant Company.

THEREFORE, for and in consideration of the foregoing premises, I hereby WAIVE,

ABANDON, and RELINQUISH all my participation, rights, claims and interests of whatever
nature arising from my previous employment and further EXEMPTING and RELEASING the
said Company, its directors, officers, stockholders, and employees from any and all liabilities,
claims, rights or actions that I may have, whether past, present or contingent.

With full knowledge of this instrument, I have hereunto affixed my signature this 24 th
day of September, 2021 in the City of Makati.
