JOINT AFFIDAVIT

We, Romulo Niñal Sr., of legal age, Filipino, married and a resident of Barangay Dos
Hermanas, Talisay City and June Rey Locsin, of legal age, Filipino, married and resident of Brgy.
Dos Hermanas, Talisay City after being sworn to in accordance with law do hereby state:

That we personally know JOYCE ANN E. SOLIGAN born on NOVEMBER 18, 2007 at
Talisay City, Negros Occidental from parents JOELITO SOLIGAN AND ANALIE ELIM;

That the child was baptized according to the rites of the Roman Catholic Church at the
Parish of San Pedro Bautista, Brgy. Dos Hermanas, Talisay City on January 25, 2009 by Rev. Fr.
Joeffrey T. Flores having Romulo Niñal Sr., and June Rey Locsin as sponsors;

That when the brother of the said child went to the office of the Parish to secure the
copy of the child’s baptismal certificate, he found out that the baptism of his sister was not
recorded in the Parish Book of Baptism;

That we attest to the truth of the foregoing facts being there present during the rites of
baptism as sponsors, and for the purpose of requesting the Parish to record the late entry of
data in the Book of Baptism of the parish and to issue the Baptismal Certificate of the child.
