MONICA: TEENAGE PREGNANCY

ASSESSMENT
1. What objective data do you have about Monica?
 Lethargic
 Tired
 Drowsiness
 Dark lips
2. What caused Monica to eat the way she did?
 When the stress got to much for her so she gained 15 pounds during her
pregnancy.
3. Which prenatal behaviors of Monica were helpful? Which where not?
 The prenatal behavior helpful for Monica is she made and kept her follow-up
visits because she wanted to have a healthy baby.
 And the prenatal behavior that not helpful is she eat too much and smoke
when the stress level got too much for her.
DIAGNOSIS
4. Complete the following diagnosis statement: Monica’s baby’s risk for
disproportionate growth was related to smoking, eating habits and stress.

as evidence secondary to smoking, alcohol and stress.

PLAN/GOAL
7. What is your plan for Monica’s health after the birth?
 After birth provide important physical and emotional care and recovery for
Monica and her baby. Trained and educate Monica about postpartum and
watch for signs of postpartum depression and, work in tandem with a
lactation consultant to assist Monica with breastfeeding.
8. What is your goal for the baby’s health-related to reducing the risk of SIDS and
delayed development?
 Goal for the baby’s health- related to reduce the risk of SIDS, newborn should
be placed for sleep in a supine position (wholly on the back) for every sleep by
every caregiver until the child reaches 1 year of age.
IMPLEMENTATION
9. What topics does Monica need to be taught about her baby’s health and
development? What does she need to know about apnea and SIDS? What does
she need to learn about normal infant growth and development?
 The topic that needs Monica to be taught about her baby is the growth and
development of newborn.
 Monica needs to know about apnea and SIDs are apnea is the stoppage of
breathing during sleep, an abnormal bluish discoloration to the skin (cyanosis)
and sometimes an unusually slow heartbeat (bradycardia). infantile apnea
may be related to some cases of sudden infant death syndrome (SIDs) it is
associated with problems in the ability of the baby to arouse from sleep and
also known as the cot death or crib death.
 Monica needs to learn about the normal infant growth and development are
by about age 2 weeks, her infant should start to gain weight and grow quickly,
by age 4 to 6 months her infant weight should be double the birth weight,
during the second half of the first year of life, growth of infant is not rapid,
and between ages 1 and 2 a toddler will gain only about 5 pounds or 2.2
kilograms.
10. Who else need to be present for the teaching?
 Monica is a teenage and a first time mom so her parents and boyfriend also
need to be present for teaching.
11. As a teenage mother with a low-birth-weight baby, Monica needs what type
of medical and nursing follow-up? Would home health care nursing visits help?
 Monica needs kangaroo mother care (KMC), it is effective method of caring
for preterm and and low-birth baby. It involves holding a newborn skin to skin
contact, day and night, prone and upright on the chest of the mother. It help a
low-birth weight newborn stability of the heart rate and breathing, lower
rates of infection and better weight gain, to prevent hypothermia and helps
establish breastfeeding.
 Home health care nursing visit can helps a mother what she should do in
caring for her newborn especially when a baby is low-birth weight.
12. What is your primary concern for the baby?
 My primary concern for the baby is the infection and breathing problems
because low-birth weight is a high risk for a newborn that may can cause of
an SIDs.

13. At the next visit, what criteria will the nurse practitioner be using to see if
Monica and her baby are healthy?
 The nurse practitioner should identify that Monica has no signs of postpartum
depression and her baby will gained enough weight and shows normal
development.

14. What other problems are likely for Monica’s baby because he was a low-birth-
weight baby?
 Difficulty of feeding and gaining weight
 Infection
 Neurologic problems, such as intraventricular hemorrhage (bleeding inside
the brain)
 Gastrointestinal problems, such as necrotizing enterocolitis (a serious disease
of the intestine common in premature babies)
15. What other issues make this situation more difficult for a teen mother to be
successful?
 Being a teen mother face many physical, psychological, social and spiritual
challenges especially in expenses in raising a child at a young age. And some
teenage parent unable to graduate because of their responsibilities as a
parent so become a teen mother it can be difficult to be successful.
16. What other types of support may be helpful to a teenage mother?
 Support of parents may helpful for a teenage mother. And also joining in
program for young parents that government provide for a teenage parents so
that they will can be more effective, responsible, nurturing mothers and
fathers.
