ACKNOWLEDGEMENTS
As I went through work from home under Faculty of Computer and Mathematical
Sciences, UiTM Cawangan Pahang, Kampus Raub as an intern from 25th May 2021 to 17th
August 2021 was a memorable one for me as it was rich in experience and knowledge sharing.
Even though the internship went through online and work from home due to pandemic, it
helped me discover my potential. I have had so many rich experiences and opportunities that I
personally believe will forever shape and influence my professional life while fostering
personal growth and development.
My internship would not have been possible without the contribution and collaboration
of others. My sincere gratitude:
• To Almighty God who granted me health and long life, without which I could not have
finished this internship;
• Sir Roger Canda, who is the Supervisor of Practical Training of Faculty of Computer
and Mathematical Sciences, UiTM Cawangan Pahang Kampus Raub. I want to thank
him for giving me the opportunity to follow my internship. He had the kindness to help
and guide me throughout my internship with advice and feedback despite his busy
schedule;
• Miss Juhaida binti Ismail, who is the Former Course Coordinator of Diploma of
Computer Science (CS110). I want to thank her for encouraging me in doing my
internship to graduate my diploma. Her guidance which was a remarkable force that
enabled me to successfully complete the internship program;

CHAPTER 1: INTRODUCTION
Practical training is a compulsory course for all students of the Diploma of Computer
Science. It is important to complete this course to complete the diploma and to graduate from
the university as a student of computer science. Training refers to the job experience that must
be related to what the student has learned from the past semester.
The main objectives for the student to perform industrial training is to give them
working experience with specific task that was given by an organization. The other objectives
of practical training are:
The student who has completed the industrial training in the given time will prepare
this report. Student must provide a computer report according to the standards given by MARA
University of Technology (UiTM). This is to evaluate the effectiveness of their skills during
the given period.
This report will also provide evidence for students who attend industrial training and as
a guideline for future use. This report, on the other hand, will also train students to comply with
the laws and regulations in their daily lives.

CHAPTER 3: ONLINE FORM BUILDER
3.1 Introduction
Online form builder helps us in creating types of form such as registration forms,
application forms, surveys, contact forms, payment forms and more. Most online form builder
offer the same set of features such as a variety of question and field options, a drag-and-drop
editor, and basic themes and templates. This online form builder helps us in saving the results
in a spreadsheet-like report, and alert us via email when we receive new responses.
3.2 Objective
i. Prices.
ii. Features.
iii. Ownership.
iv. Advantage.
v. Disadvantage.
➢ It works in the same way as Google Forms, but with more data analysis
functionality thanks to its integration with Excel. Adding questions to Microsoft
Forms is fast and simple. It offers response types that we could not find in
Google Forms, like Net Promoter Score. We can send people to a direct link to
respond then get a quick overview of your responses within the Microsoft Forms
interface or in an Excel spreadsheet.
8
Figure 3.3.1: Logo of Microsoft Forms
3.3.2 Typeform
➢ It is a uniquely designed form, showing one question at a time and blurring out
the others, making the overall form submission process feel very conversational
and intimate. Its form can include cover pages, paragraphs of text, and
multimedia along with traditional form fields. It has oversized buttons that
makes it easier to use on a touchscreen, so it is one of the best options if we
want our form to look great on mobile.
3.3.3 Wufoo
➢ It was one of the first form tools to make online forms look good. It is part of
SuveryMonkey, one of the earliest web apps with a history dating back to 1999.
It makes up with extreme customizability. It lets us build a form that is
completely unique. It has a powerful report generator that is simple to use.
9
Figure 3.3.3: Logo of Wufoo
3.3.4 JotForm
➢ Its form is much more customizable than forms created on Google Forms and
Microsoft Forms. It lets us do things like accept payments and collect signatures
without upgrading to a premium plan. It stands out with its immense template
library – we can choose from more than 10,000 form templates.
3.3.5 Formstack
➢ It may be the best option for company that is in a highly regulated industry. It
offers specific HIPAA- and GDPR-compliant forms that keep respondents’
private data secure. It will warn us if our form does not meet Section 508
Compliance standards – for example, it would not work with assistive
technologies like screen readers.
10
Figure 3.3.5: Logo of Formstack
3.3.6 Paperform
➢ It is a form for gathering data. It is designed to look more like landing page or
blog post rather than standard list of form fields. Add photos, headings, video,
and long-form text can be used in our form. It is a nice substitute for a landing
page builder, lets us collect visitor data on custom landing pages for things like
PPC ads and targeted marketing campaigns.
3.3.7 Formsite
➢ It lets us encrypt the text in specific form fields so that responses appear
unintelligible to people without permission to view them. It comes with built-in
payment processing integrations like PayPal, Stripe, and Authorize.net, or use
encrypted fields to directly accept credit card or ACH data through our form.
11
Figure 3.3.7: Logo of Formsite
➢ It is a WordPress plugin used originally for contact forms, but in a more general
sense. It allows site owners to create forms to collect information. Can be used
for contact forms, WordPress post creation, calculators, employment
applications and more. It has a beautiful design form editor, intuitive drag-and-
drop column control, and robust accessibility improvements.

CHAPTER 4: GOOGLE SITES
4.1 Introduction
Google Sites is a free website builder from Google. It allows us to create website with
collaborators by giving another Google user edit access. It is compatible with other Google
services like Docs, Sheets, Forms, and Slides. It uses your Google Drive account to store
images and files we share on our site up to 15GB of files for our sites for free, shared with our
other Google apps.
4.2 Objective
To create a website using Google Sites by referencing a sample website given that
includes the usage of Google Forms.

CHAPTER 5: CONCLUSION AND RECOMMENDATION
5.1 Conclusion
Overall internship is a good program. It helps to enhance and develop skills, abilities,
and knowledge. It was a good experience and memories as not only I have gained experience,
but also a new friends and knowledge. With the help of Sir Roger Canda, I managed to do my
internship at work from home due to pandemic. It was a tough journey as I need to do
everything through online. I learned the different facets of working during this pandemic.
I am grateful that I have managed to complete the industrial training for 3 months
starting from 25th May 2021 to 17th August 2021. Sir Roger Canda gives me a lot of experience
and memories to be remember. I have gained experience that I never expected such as intern
at work from home. It has improved my confidence level to adapt with new working
environment due to pandemic.
All in all, I would like to thank my supervisor, Sir Roger Canda for giving me
opportunity to do my internship and guide me to complete my task. All experience that I gain
there is extremely valuable, and I may not get it anywhere.
5.2 Recommendation
i. To the university, I would suggest to secure internship placements for students. This is
because some students could not find place to do internship.
ii. Increase the internship period. The university should at least adjust on the period
students go for internship. The period should be at least 4 months and above.
