I. Teaching and Learning Process
1. Prepared and submitted 20 Daily Lesson Logs in all subjects taught with
complete parts, appropriate, adequate and signed by the SH;
2. Prepared, made and utilized 10 charts/pictures and worksheets in English 5,
English 6 and Aral Pan 5;
3. Administered the Third Quarterly Examination, PHIL-IRI, and Numeracy Test to
the pupils;
4. Submitted and kept updated school and pupils’ records such as Class Program,
Monthly Target and Accomplishment Reports, Reports on SBM, Updated Form
138 and issued Report Cards quarterly, E-Class Record/Progress Record, SF 2
and other required school/pupils’ reports;

II. Learning Outcomes

1. Improved Literacy/Numeracy Level of Grade V pupils by conducting
Remedial/Enrichment Reading Instruction;
2. Posted Monthly Reading Progress Chart of the pupils;
3. Coached/Trained pupils in the following:
- District Communication Arts Festival in English and won 1 st place in Spelling
Contest in Grade 5 & 6 and 1st place in Oral Interpretation in Grade 5;
- Sectoral/Congressional Communication Arts Festival in English and won 3 rd
place in Oral Interpretation in Grade 5;
- Division Communication Arts Festival in English and won 5 th place in Oral
Interpretation in Grade 5.

III. Professional Growth and Development

1. Constructed Periodical Test Questions in English V & VI and Filipino V & VI
utilized by all Grades V & VI pupils in the District;
2. Checked the DLL of co-teachers in the absence of the SH;
3. Performed the duties and functions as OIC of the school;
4. Served as facilitator in the District DepEd MTAP Challenge;

IV. Community Involvement

1. Maximized community involvement in the School Rabuz Work especially in the
school rehabilitation after Typhoon Nina;
2. Conducted meetings with the parents and other school stakeholders like 4Ps,
alumni, and LGU officials to disseminate and maximized their participation in the
implementation of school programs such as School Feeding Program, Gulayan
sa Paaralan, Greening andTtree Planting.
3. Served as Zone Coordinator in the barangay and in the implementation of
community programs.
