Confirmation on the Receipt of the FY 2021 DRRAP

Dear Director Timbreza:

This is to confirm the receipt of the Municipality of CAPAS the amount of 1,722,336.00 through
the Notice of ADA Issued (NADAI) No. 990-21-02-011 dated February 24, 2021 intended for the
implementation of the following projects under the FY 2021 LGSF-DRRAP Program:

The said funds have been transferred from the General Fund to the Municipality’s Trust Fund
Account including the MLGU counterpart mentioned-above, if any. Further, a separate subsidiary
ledger/s for each project is/are being maintained. Attached is a certified true copies of proof of
fund transfer.

Rest assured that we are committed to implement the above project/s in accordance with the FY
2021 DRRAP Guidelines LBC No. 134, Series 2021.
