REQUIREMENTS FOR LGU ELIGIBILITY:

Recipient of Department of the Interior and Local
Governments (DILG) Seal of Good Housekeeping (SGH)
for CY 2014 Program Implementation or Good Financial
Housekeeping component of the Seal of Good Local
Governance FY2014 Assessment for CY 2015 project
implementation;
Submission of the PFM Improvement Plan (for 2015
focus LGUs under the LGU PFM 2 Project) or initiation of
its implementation (for 2013 and 2014 focus LGUs
under the LGU PFM 2 Project), as determined by the
DBM Regional Office; and
Certification by the Accountant that funds previously
transferred, if any, has been liquidated, post audited
and accounted for in the books. Partial liquidation may
also be accepted as warranted by the Head of the
Office thru the Accountant.

Appropriate Sangguniang Resolution (SB): (i)
Authorizing the Local Chief Executive (LCE) to enter into
Memorandum of Agreement (MOA) with DSWD-FOs: and
(ii) Approving the List of Priority Poverty Reduction
Project(s) and corresponding amount(s) including LGU
Counterpart Fund(s)
MOA between the DSWD and the LGU
Program of Works
Detailed Engineering Estimate/ Design/ Plans and
Drawings
( or) DOD, Usufruct Agreement or Certificate of Land
Availability (CLA) for infra projects, w/c ever is
applicable (for new projects)
( or) Certificate of Non-Coverage or Environmental
Compliance Certificate (For Sub-Projects that concerns
Social Safeguard)
( or) Municipal / Barangay Resolution/ Certificate of Existing
Usage of Project Site (Barangay & Municipal Owned
Site)
