What happened

Surplus pipes (18-in diameter, 12m long) rolled-over from their supports. The pipes which had been stored for almost 8
years, were stacked 4 rows high with wooden stops at either end. There was no one in the area at the time of the
incident.

Immediate causes: FDXVDVLQPHGLDWDV


The rollover happened when the nailed wood pipe stops failed due to corrosion (of the nails) and weather. In addition,
the pipes had a rather smooth surface and were stacked too high, increasing the risk of rollover.

Root causes: &DXVDVIXQGDPHQWDOHV


• Lack of a procedure for the long term storage of pipes including the type of stops to use and stacking height.
• No maintenance and inspection plan implemented to detect and repair defects.

• Establish and Implement a procedure for storage of pipes including :


o Maximum storage height, number of rows and storage configuration (to ensure that pipes are
arranged in a configuration that will reduce the risk of rollover)
o Roll over protection systems required (e.g. in case of long term storage, use rubber stops)
o Roles and responsibilities for maintenance and inspection of the storage

• Ensure that both short-term and long-term storage are subject to risk assessment taking into account all
foreseeable hazards
