INCIDENT REPORT

Date and Time of Report: 08:30 Saturday, 13 of March, 2021

Location: Working area, behind warehouses, the pipes storage location close to West Gate of BUT Camp

Detailed Description of Incident
08:30 received a message from Saadik Khamil (Sinhareeb Co.) about some suspicious object
was found in the working area behind warehouses.
08:35 arrived on scene, established “4 C”( Clear, Cordon, Communicate, Control). Cleared the
area close to UXO from people, marked the exact place of UXO and appointed 02 Security
Guards to prevent anyone from crossing the cordon (Red Tape).
08:38 SOD was notified.
09:00 Received phone call from Mr. Han about confirmation of arrangement of UXO team.
09:40 UXO team has arrived and cleared the UXO. That appeared to be just a cover from the
old land antipersonnel mine.
09:55 Reported to SOD about clearance of the UXO.

Risks Identified
UXO are common in Iraq. There is a possibility that in this area there are a lot of objects can be
hidden deep in the ground due to prior war between Iraq and Iran. Security on site can provide the “4
C” procedures – Clear, Cordon, Communicate and Control of the approximate area of UXO.

Lessons Learnt and Corrective/Preventative Steps Envisaged


As long as it is common situation, more attention should be kept on the working places. All suspicious
objects supposed to be marked and reported to Security on site.

Additional Relevant Information

The UXO team arrived approximately in one hour after the incident was reported to SOD.
