1. OBJECTIVE

To minimize the damage to persons, the environment, and equipment caused by


accidents or incidents that can occur on the site during its construction and
commissioning.

To reduce the risk to the health and safety of all those affected by an emergency at
the site.

To be applied to all personnel present on the site, contractors, subcontractors,


providers, visitors, and to any work carried out in the MESAIEED A POWER PLANT
AT QATAR.

2. LOCATION OF THE SITE

The IBO site is located in between Qatalum and Gabro port area.

The following map included the main access to the site.

3. MUSTER POINT LOCATION

The whole installation is divided into distinct zones. The zones have been given
numbered numerically & alphabetically. Installations of muster points are included in
each area.

The whole site is divided into distinct areas.

All evacuated personnel will gather in the nearest muster point so that head count
and assessing the number of the personnel will proceed. Assembling other than the
muster point is prohibited to avoid these personnel from going to zones or areas
where their presence could hinder the extinguishing and evacuation efforts.

4. EMERGENCY ACTION PLAN

4.1 Levels of Evacuation

4.1.1 Emergency Outbreak

These are situations that can be neutralized by available means within the affected
area and for the personnel stationed on it.

Typical incidents that may cause these emergency situations are the following:

 Small leak or escape of toxic or combustible products.

 Fire of a vehicle or machinery.

 Fire in equipment of minor importance.

Key personnel to be activated are:

 ERT Coordinator

 ERT members on the area

4.1.2 Partial Emergency

These are situations requiring in addition to the ERT, the collaboration of many staff
as necessary without the need for evacuating the rest of the personnel. More than
one group can be activated on this type of emergency.

Typical incidents that may cause these emergency situations are the following:

 Any fire that cannot be controlled in the immediate vicinity but that can be
controlled with the means existing in the work site and does not require
external intervention (Fire Department, Civil Protection department).

 Minor oil spill, leak of toxic or combustible products affecting an installation of


the work site.

4.1.3 General Emergency

These are situations requiring the intervention of all the human and equipment
resources of the site including when necessary the participation of external
organizations.

Typical incidents that may cause these emergency situations are the following:

 Uncontrollable fire capable of affecting the activities of the work site as well as
those outside it.

 Combustible liquid leak

 Inflammable gas leak

4.2 Definition of Responsibilities

In an emergency situation, the chain of command will be formed according to the


chart in Appendix I and the personnel involved is named in Appendix II.

4.2.1 Emergency Director

This is the responsibility of the IBERDROLA Site Director.

 To lead necessary efforts to avoid or limit the consequences of an


emergency situation within and outside the limits of the site.

 To activate the Emergency Plan (Activation of the Emergency Groups)


as well as the partial or total evacuation of the site personnel.

 To order the help of external assistance.

 To declare the end of the emergency and write the Emergency Report.

4.2.2 Shift Manager

 To inform the Emergency Director of any anomaly that could result in


the activation of the emergency plan. Should an emergency outbreak
exist, he will attempt to control the situation and neutralise the
outbreak with the means and personnel available.

 To assist at all times the Emergency Director on the actions to take,


given his knowledge at all time of the current state of (equipment in
operation, authorised tasks being carried out, etc).

 Follow at all times the instructions given by the Emergency director


and transmit them to all the site personnel that he deems necessary.

4.2.3 Emergency Response Team Coordinator

 Coordinate all activities to be carried out by the distinct Emergency


Groups.

 Continually assist and inform the Emergency Director regarding


decisions taken.

 Instructs Muster Point Coordinators for an immediate evacuation when


instructed by the Emergency Director.

4.2.4 Emergency Response Team (ERT)

 The First Intervention Group is responsible for being the first to


respond to a declared emergency in a specific site. This group will be
managed by the ERT Coordinator.

 The main function of this group is to locate, rescue, and apply first aid
to affected workers for the accident/incident and organise all to do with
their medical evacuation.

 The ERT are trained for basic First Aid, Fire Fighting and Confine
Space Rescue following the ratio of 1:25 for each contractor. The 50%
from the total number of these personnel are trained for the advance
First Aid and Fire Fighting courses.

4.2.5 Muster Point Coordinator

 Supervises the evacuation of all personnel on his area or zone.

 Receives reports from each contractor’s ERT members about the


status of their personnel and areas. The contractor’s ERT members
need to give complete information about the personnel and worksite on
the area concerned.

4.3 Actions to be taken by the ERT members when detecting an emergency.

In order to avoid or reduce the consequences of an incident and/or accident, it is


extremely important that:

 Actions are taken rapidly and efficiently.

 Instructions are followed.

 Information and communication are adequate and complete.

Therefore it is fundamental for the person detecting an emergency to act in the


following manner:

4.3.1 Give the alarm notification immediately by telephone (mobile) or personally


giving information but not limited to the following:

 State the name and this is an emergency call

 Nature of emergency

 Location

 Services/support required

 Site machinery or agents involved

4.3.2 Inform the following key personnel:

 Medical services.

 ERT Coordinator

4.3.3 Maintain calm.

4.3.4 Not take unnecessary risks.

4.3.5 Use available means (extinguishers, fire hoses).

4.3.6 Assist accident victims.

4.4 Actions to be taken by personnel not belonging to emergency groups

All persons not having an assigned function in the Emergency Plan (including visitors
and suppliers) will be required to act in the following manner.

4.4.1 Free all phone lines in the office.

4.4.2 Remain attentive to any instructions given by the emergency director.

4.4.3 If there is an evacuation, go quickly and in an orderly manner using the safest
route to the Muster Point indicated by route map (site access control).

4.4.4 Remain in the indicated Muster Point until instructions are received from the
Emergency Director.

4.5 Emergency Control Center

The emergency control center is located at the site office specifically at the site
manager’s office. It is equipped with the technical means to maintain necessary
internal and external communications (fax, email, telephone).

The emergency control center is the gathering point for the emergency director, shift
manager and emergency response coordinator.

A folder with the emergency plan and the general site plans is kept in the center.

4.6 Alarm system

Manual alarm systems are installed in both the IBO office area and site location for
quick and immediate notification of an emergency. Authorization is given by the
Emergency Director and in his absence by the ERT Coordinator once the need to
evacuate arises.

4.7 External means

Telephone numbers for the civil defence authority and the closest Mesaied Medical
Center (MMC) hospitals are indicated in appendix.

4.8 Activation of emergency plan


Once an alarm is received and confirmed, the Emergency Director will order the
activation of the Emergency Plan.

1. Its activation implies the notification either verbal or by telephone of


organisations and persons with roles in the Emergency Plan. He will request
from access control that relevant acoustic signals be sounded and will deliver
the messages he deems appropriate.

2. In the case of a partial emergency, upon activation of the Emergency Plan,


the First Intervention and Assistance groups will gather to the vicinity of the
EMERGENCY SITE to receive instructions.

3. In the case of a general emergency, upon activation of the Emergency Plan,


all ERT members will gather in the vicinity of the EMERGENCY SITE to
receive instructions.

5. IMPLEMENTING THE EMERGENCY PLAN

Once the Emergency Plan is written it is necessary to proceed with its


implementation, which comprises a number of measures destined to ensure the
operational status of the Plan.

Necessary activities in the implementation phase are as follows:

5.1 Education, information and training activities

Persons who are members of the Emergency Response Team which meets the ratio
of 1:25 per contractor are trained in basic first aid, fire fighting and confine space
while 50% of these personnel are trained for the advance. Once the plan has been
defined it is necessary to establish a training program destined to disseminate and
clarify the said plan. Distributing in writing the concrete orders to all constituent
members and exposing at the individual level the duties of each team in case the
emergency materializes is important.

In parallel, training programs will be established for the site’s personnel in general,
organising informative reunions for all the personnel and distributing guidelines that
will contain information on at least the following points:

 Precautions to adopt to avoid causing an emergency.

 Way to communicate the detection of an emergency.

 Manner in which the alarm and evacuation order will be given in case of an
emergency.

Information on what must be done and not to be done during an emergency.

5.2 Drills

The function of said drills is to test the effectiveness of the designed plan and to allow
the detection and correction of errors and omissions in it. The drills will provide
practice and confidence to all the personnel in the work site (as much the workers
with specific duties in case of emergency). The drills in addition will serve to estimate
the various teams’ evacuation and intervention times.

The drills will be conducted at least twice a year to keep the ERT prepared and ready
for any eventuality. The emergency plan will be reviewed based on the outcome of
the drill or in case of the need.

5.3 Emergency Calling Numbers

A calling card will be made and kept always available in the Emergency Control
Centre.

This card contains the contact numbers and addresses that could be necessary in
case of emergency. Information in the card will be regularly updated.

5.4 Maintenance of Equipment

An agenda for the maintenance of detection, alarm, and extinguishing devices will be
kept.

The frequency of the inspections will be at least in keeping with the program
established by the Fire Protection Installations Regulation.

o Fire protection inspection.


o Portable extinguishers inspection.
o Fitted fire hoses and hydrant conditions inspection.
o Fire protection inspection report (archiving any anomaly detected and the
action taken to redress it).
o Risk report.
o Fire investigation report.

6. MAINTENANCE OF THE PLAN

6.1 Responsibilities

The responsibility for the Emergency Plan falls on the party responsible for the
activity. Therefore, management personnel, middle management, specialists, and
workers are obliged to participate.

6.2 Human Resources

See appendix I of this document.

7. MAINTENANCE OF THE FIRE PROTECTION INSTALLATION.

The party responsible for maintenance and the user will keep records of fulfilment of
the preventive maintenance programme indicating at least: operations carried out,
inspection results, and proof of any substitution of defective elements carried out.
Annotations will be kept up to date and made available to inspection services.

Operations to be carried out by the party having manufactured or installed the system.

will be stamped, in accordance with section ITC-MIE-AP.5 of the
Pressure Devices Regulation on fire extinguishers (Official State
Bulletin nº 149. 23 June 1982).

emergency and that visual panel is intact and firmly mounted in its frame, that locking
exits functions properly and allows complete closing of the door, that latch is
correctly aligned and maintains door closed, that closing selector in double
panel doors is correctly attached and overlaps the doors correctly.
Adjustment of closing springs. Substitution of broken, damaged, or non-
working components of the anti-panic bars, verification that they work
correctly and allow the complete closing of the door.

Annually Verification that fittings are correctly attached to door and frame. Verification
that there is no corrosion in metallic material doors. Greasing of guides,
hinges, anti-panic bars, springs, etc.

Showers and Weekly Accessibility verification. Signposting verification. Correct functioning

APPENDIX IV DURING EMERGENCY


IN ORDER TO FACILITATE THE CIRCULATION OF ALL EMERGENCY VEHICLES,
ALL ROUTES AT THE SITE MUST REMAIN FREE OF OBSTRUCTIONS.

When an alarm sounds or notification is given, all non-emergency vehicles must park
in a safe place as soon as possible where they shall not constitute an obstacle.
Motors must remain turned off, but with ignition keys in starters.

Access Control shall not allow any individuals or vehicles into the area unless they
are Iberdrola personnel, or they belong to external support services (Emergency
Medical Assistance Units, Civil Protection, or Civil Guard). They shall be responsible
for maintaining the principal access or exit completely clear of vehicles at all times.

Medical Assistance Unit at the site: they shall always follow the instructions issued
by the Emergency Director and the Emergency Response Coordinator.
