INCIDENT DURING RADIOGRAPHY

1.Purpose

An ERAP has been developed by M/s Tuaman Engineering Limited to provide a formal emergency
preparedness program that protects human health and property in the event of an emergency such as
fire, public disturbance, possible terrorist threat / attack, natural calamity etc. This plan also formalizes
the arrangements with local police, armed security forces, fire departments, hospitals, and emergency
response contractors, in case of an emergency.

2.Scope: -

Emergency plan shall be communicated to all the staff & employee of IOCL projects. This is
not covering any unforeseen incident/ events where the mentioned program is not enough to
cover the risk

3.Objective: -

The main objective of emergency plan is to minimize the risk to men & machinery and
rapidly control the damage to minimum & reduce recurrence.

The emergency can be of below mentioned type but not limited to:-

Fire and explosion


Spillage and Toxic chemicals & acid
Total and Serious power failure
Serious accident situation causing fatality or multiple injuries to human life
Natural calamities i.e. flood earthquake cyclone

Medical emergency like food poising, water poising etc


4 Roles and Responsibilities

Primary responsibility of declaring emergency at site is with Resident Construction Manager of that
project site. He is responsible for implementing emergency response action plan and identifying
individuals who are trained to coordinate response to emergency events. These persons will be in
charge of directing activities during emergency situations by implementing the ERAP. During
emergency at site the responsibilities of each level of management and staff are described in the
following sections.

4.1 Site Main Controller

Site main controller shall be M/s Tuaman Engineering Limited Resident Construction Manager. He will:

 On consultation with the Incident Controller, decide whether a major emergency exists and on
declaration of a major emergency declare by client and ensure that the outside emergency services
and mutual help are called, the off-site plan activated and if necessary, nearby factories and
population are informed.
 Ensure that the key personnel are called in and all M/s Tuaman Engineering Limited employees
are aware of the emergency situation.
 Direct the safe close down and evacuation of site in consultation with the Incident Controller and
key personnel.
 Ensure that casualties are receiving adequate attention. Arrange for hospitalization of victims
and additional help if required. Ensure that the relatives are informed.
 Inform the fire, armed security forces and police services, district emergency authority etc.
 Site team shall remain alert at all times for immediate evacuation

 Ensure the accounting for personnel and rescue of missing persons
 Control traffic movement within the project site. Thorough screening of the vehicles at all entry
points shall be done. In case of any unidentified vehicle movement notified by security forces, he will
alert project team immediately.
 All the workers entering and leaving needs to be thorough screened for arms and weapons and
metal detectors shall used for screening purpose.
 Arrange for a chronological record of the emergency to be maintained.
 Ensure that proper consideration is given to the preservation of evidence. Arrange for
photographs/video.
 Control rehabilitation of affected areas and victims on cessation of the emergency. Do not restart
the site unless it is ensured safe to start and cleared by the authorities.

4.2 Incident Controller

Incident controller shall be M/s Tuaman Engineering Limited Deputy Site Manager or safety Manager of the
project site.

 Take charge at the scene of incident and assess the scale of emergency.
 Decide for continuing the work.
 Ensure that outside emergency services have been called in
 Ensure that Site Main Controller (SMC) and key persons are informed.
 Direct essential workers to carryout their duties to control the situation and to mobilize the
resources.
 Evacuate non-essential workers to assembly points.
 Brief the Site Main Controller and keep him informed of the developments.
 Preserve evidences for subsequent fact-finding investigation.
 Ensure that the unaccounted persons are searched .
 Ensure the readiness of all the emergency system such emergency vehicles, communications
system, emergency appliances and security services to handle the situation.

4.3 Site HSE Co-ordinator

It is the responsibility of the HSE Manager or Site HSE Coordinator to:

 Report to Incident Controller at the Incident spot.


 Guide and assist to the Incidence controller in combating emergency.
 Arrange required PPE, required number of vehicles as per size of the site team and other
emergency appliances.
4.4 Key Personnel

Key personnel are those who are identified for doing specific task during emergency
e.g. Doctor, Rescuer, Security Commandant In charge of the plant etc. They will:

 Perform their respective prescribed duties to help control emergency.


 Act as per directives from Site main controller or Incident controller.

5.0 EMERGENCY RESPONSE PLAN COMPONENTS

5.1 Training

Training in emergency response includes:


 Emergency Preparedness
 Knowledge of the site evacuation plan
 Effective utilization of safety equipment and communication devices.
 Emergency medical training including first aid and CPR (Cardiopulmonary Resuscitation).
 Fire fighting
 Safe places for assembly Local emergency response agencies are advised of training sessions
at the facility when appropriate and invited to send representatives.
 Employees should be given fire, site emergency & evacuation drill at least once annually.
 Nominated first aid personnel should be certified in both First Aid and CPR.
 Doctor shall be nominated to handle all the medical emergencies for the site.
 All new employees will be trained in emergency procedures as part of their orientation.
 All training will be documented, and records will be kept.

5.2 Communication

All personnel involved in the project shall be aware of emergency alarm system and appropriate trainings
and mock drill on this regard shall be conducted.

5.3 Meetings

The site ERAP team should meet on the following occasions:

 Any emergency arises


 After emergency settles
 Pre-Mock drill meeting
 Post Mock drill meeting
 Periodic meeting to implement ERAP

5.4 On Site Response Equipments

A wide variety of primary emergency response equipment is maintained at the facility in a state of operational
readiness. Employees should know the location and use of this emergency response equipment. All
response equipment is to be tested and inspected by qualified personnel at least once annually.
The following information/equipment should be kept in readiness:
 Site layout showing assembly points
 List of employees’ blood group
 Telephone directory –Internal and External (Attachment-C)
 Telephone directory –ICE (In Case of Emergency) contacts details of the dependents of all site
staff
 Portable fire extinguishers, including A, B, and C class units

 Personnel protective equipment
 Emergency medical equipment
 Vehicle for medical facility and evacuation
 Packet Radio sets for latest news update
 Torches
 Digital Camera
6.0 Scenario details of ERP:-
Where specific procedures may not have been established judgment shall be used in determine the best
course of action. Follow the guideline below for assistance: -

 All emergencies are to be handled by the highest-ranking person present and assisted by the
emergency squad.
 The highest-ranking person shall assist job to responsible person for making emergency calls.
 The emergency phone numbers are to be displayed at the entrance gate and assembly point.
 The need of the ambulance or other emergency equipment shall be determined by the highest-
ranking person present.
 Medical person present, except where a catastrophic event has occurred, on that time anybody
can call for an ambulance.
 In all instances the top mgmt. shall be notify immediately by phone / electric motor siren.

6.1 Fire protection procedure: -

General fire protections procedures describe in this section apply to any location emergency procedure to be
used in the event of fire include the following,

 Make a safe attempt to extinguish, do not endanger your life.


 At the same time notify the admin officer, fire dept., safety officer.
 Have an employee meet the fire dept. at the entrance of the building to direct them location of the fire.
 Keep all the spectators and non-essential away from the fire.
 If explosives type materials are involved, immediately evacuate all personnel.
 Make no comments refer all inquiries to the admin manager.
 Security guard will be instructed in the use of portable fire extinguisher, first aid response equipment
and will be thoroughly familiar with plans for calling local units to fight fire after working hours, on
Sundays and holidays.

6.2 Accidents involving serious injury

 Barricade the area/do not disturb/distort the accident scene.

 Provide for necessary first aid to the victim if necessary.

 Call ambulance or have first available person call nearest hospital services.

 Notify to the mgmt.

 Remove all non-essential persons.

 Provide assistance to rescue personnel as required.

 Allow no picture to be taken, expect upon approval of the admin officer.

 Make full investigation and report as per the requirement of the safety management system.

6.3 Property damage accidents

 Notify the admin officer at the first, if available. The admin officer should notify immediately or arrange
notification to the mgmt.

 Protect against further damage whenever possible.

 Wherever the possibility of fire, electrical injury or explosion exits take additional measures as
necessary to protect.

 Keep all spectators and essential employees back and or away from the area.

 Make no comments refer all inquiries to admin officer.

 To the extent possible, allow no one to take site pictures, except upon the approval of the Admin
officer.

6.4 Public demonstrations

 Notify the admin officer who will notify appropriate authorities.

 Remove all the employees from the demonstration area.

 Do not allow any verbal or physical conformations, except for personal protection.

 If the condition become worse and no supervisory personal are present employees are to remain in
the office await further instructions.

7. Response preparedness

Appropriate and practical response activities shall be initiated in the event of any incident, which results in
fire, explosion, possible terror threat /attack or other emergency.

7.1. Evacuation plan


Evacuation of the facility may be ordered based upon the judgment of the Site Main Controller or at
the request of authorities.
 All employees shall evacuate the plant and surrounding property upon learning of a fire or
explosion or other emergency by the fire alarm or oral communication.
 Personnel should (only if time and personal safety permits) shut down equipment and systems
or processes
 The evacuation routes and assembly points shall be identified and published.
 Identify the nearest hospital which handles trauma and emergency care facility and also assess
the time taken to reach the medical facility.
 Vehicle used at site shall be without corporate logo/identification to maintain a low profile.
 Upon evacuation, all employees shall report to the designated assembly points and will remain
there until advised otherwise

 A head count will be taken immediately after the evacuation. Any discrepancy shall be reported
immediately to the Incident controller or safety officer and appropriate emergency response personnel.
Under no circumstances is an employee to go back inside the plant after a missing person.

7.2 Re-occupancy of the facility

The determination that the facility may be safely re-occupied will be made by the Site Main controller
in consultation with responding emergency service agencies. Facility activities will resume only after
the Emergency Coordinator has given approval.

7.3 Notification

Notifications must be made to all the necessary parties to ensure the company, the employees, and
the incident are handled in the most effective and efficient manner. As a minimum, the following might
be contacted (depending on severity/risk): emergency responders, security team, key operations
personnel, HSE department, family of those injured, client etc.

9. Siren Code as per IOCL-PDR:


FIRE: Siren shall be wailing sound for 2 minutes.
DISASTER: Siren shall be wailing sound for 2 minutes repeated thrice with a gap of 1minute.

2.0 An effective communication system through WLL cordless phone will be established and
maintained to allow instant contact between all levels of site staffs, subcontractors especially
emergency departments for assistance.

3.0 An emergency telephone contact list of the key site staffs and emergency departments will be
displayed at site as well as office.

4.0 Emergency equipment such as first-aid facilities are to be inspected and maintained by the Safety
in charge and the First Aider on a regular basis.

11. Do’s and Don’ts during emergency: -

14. Do’s & Don’ts of COVID 19

Do’s: -
 To maintain personal hygiene and physical distancing.
 To practice frequent hand washing. Washing hands with soap and water or alcohol-based
sanitizer. Wash hands even if they are visibly clean.
 To cover your nose and mouth with handkerchief/tissue while sneezing and coughing.
 To wear Nose mask & Hand gloves.
 To maintain a safe distance from persons during interaction, especially with those having
flu like symptoms.
 To take their temperature regularly and check for respiratory symptoms.

Don’ts: -
 Shake hands.
 Have a close contact with anyone if you are experiencing cough and fever.

 Travel here and there unnecessarily, particularly to any affected region.
 Participating in large gatherings and sitting in groups in canteen.
