A. Project Title
• Write the proposed project title.
• Creativity is highly encouraged. Make the readers interested to read further.
• May include the project logo.
• This is a business name. This is not a community-based project title.
• It must be catchy. It must be short, and it must be remembered / recalled.

C. Executive Summary
• Write an introductory section, called the executive summary, to summarize your project. Just like the
introduction of an essay, this section should aim to catch your recipient's attention and encourage them to
read on. Your executive summary should include details about:
o The problem your project solves
o How your project solves the problem
o Your project's intended impact
• The length of your executive summary will depend on your project's complexity. Three paragraphs will be
sufficient for many projects, but you may need a longer executive summary if your project is more complex.
Make your executive summary concise to keep it engaging and convincing.
• Just like the abstract of a research.
• A very short discussion about the project (kung ano ‘yung project title mo and ano ‘yung general framework
niya).
• This is not a full-blown paper. More likely 2-3 paragraphs.

I. Project Background
• Write a section that provides a background to the project and the problem it seeks to address. This section
should include:
o A history of the problem (overview of the public health problem it solves) as it relates to your business
o Project detailed description
o A concise summary of your project's requirements
• Detailed part of executive summary or detailed explanation of the project.
• The ending of the project background must be “what is the face of the project?” (After basahin ‘yung project
background, makita dapat ‘yung mukha ng project natin).
• The end goal of project background is that the panel already know the framework of the community-based
enterprise.
• How did you come up with this project?
o Overview of the public health problem it solves:
▪ Inconvenience in certain areas of the community.
▪ Lack of supplies in hospitals.
▪ Limited/few equipment or materials that are available in the community.
• Difference:
o Executive summary: pahapyaw pa lang
o Project background: you will now discuss here in details. A detailed explanation of the project
• Included also in here is the framework of the project.
o What are the steps you will do in the project enterprise?
o For example:
▪ What are the services you are offering?
• Example: you will build a wellness clinic – what is the location, what are the services or package
of services that will be offered?
o For example services include: lahat ng pupunta dun na client is a-assess ko muna, after ko
silang inassess, gagawa ako ng package - meron ba yang dietary, exercise, or monitoring?
Include the details.

II. Implementation Mechanisms

• Write a section that outlines your project in greater detail. This section should explain your solution to the
problem and how you will execute it. It should include:
o Mission, Objectives, Project Scope, Timeframe of Implementation
o Projects Phases (detailed description of phases and activities/ sub-activities) in either of the following
presentation:
▪ Pre-Implementation, Implementation, and Post Implementation
▪ Milestone-based
▪ Thematic
▪ Visual representations (can be in the appendixes or in the main doc)
o Monitoring and Evaluation (a. activity-based and b. indicator-based)
• How will you build the business? Paano mo siya gagawin? The context of the community-based enterprise is
from inception to establishment.
• Difference:
o Project background: the framework of the business
o Implementation mechanisms: Paano mo itatayo ang business from inception to establishment.
• The implementation mechanism depends on the community-based enterprise (what is the applicable?)
• The ending of the implementation mechanisms must be clear on how the enterprise will be established.

Monitoring and Evaluation

A. Activity-based
• How will you monitor the implementation?
• What activity will you do to monitor from inception to establishment?
o For example:
▪ Will you do weekly meeting on the project team?
▪ Gantt chart (hypothetical) – will you update it weekly?
▪ Will you do quarterly courting meeting?
• This is your call. These are activities with the aim of monitoring the progress.

Activity Description Frequency Remarks

B. Indicator-based
• Remember that this is from inception to establishment.
• Output – lahat ba ng nasa Gantt chart mo ay nagawa mo?
• This is about the processes and activities conducted. Granted na nagawa mo sila, so mino-monitor mo
lang.
o Examples of output.
▪ Number of meetings conducted
▪ Number of FGD conducted
▪ Number of policies created
▪ Number of plans created

Frequency of
Indicator Definition Remarks
Monitoring
Output (activities)
Kung nagawa ba as
planned ‘yung activity?
Outcome
‘Di ba ‘yung
milestones/deliverables
mayroon silang mga
completion criteria. For
example, after mong mag
meeting with your core
team, baka nakagawa
kayo ng isang plano

Output natin doon sa

activity, for example,
gumawa kayo ng
packages of services
ninyo – that would be an
outcome
Impact
Supposedly eto ‘yung mga
disease entity or public
health programs like for
example, ang impact
niyan ay nakababa ba ‘yan
sa NCD? Adolescent and
teenage pregnancy?
Nakaimprove ba ‘yan sa
nutrition rate? – that
could be an impact
indicator. Pero…

Ang isang impact

indicator na pwede sa
ating community-based
enterprise kase ‘yung
context is from inception
to establishment is kung
na establish ba ‘yung
business in general?

Until ma-establish ‘yung

business or pwede ring
maglagay ng possible
impact niya doon sa mga
public health program
like for example kapag na-
conduct mo siya o kaya
kapag naging effective
siya. Example: mag
d’decrease and NCDs, mag
d’decrease ang prevalence
ng overweight pero
additional lang ito in such
cases na gusto mo lang
ilagay. Hypothetical lang
ito. Even ‘yung costing is
hypothetical kailangan
kasi natin ng costing sa
sustainability ‘di ba? Thou
hypothetical ‘yon, ma-
a’anticipate mo na siya.
Even the risks are
anticipated ‘di ba? So you
are foreseeing the
negative na baka hindi
siya mag work.
* Remember that output, outcome and impact is not required. It is suggested to put it until the activity-based. Dapat
makita talaga kung paano niyo i-m’monitor ‘yung progress nung business.
o Risk Identification and Mitigation (can be narrative, tabular, and/or both)
▪ Not required pero suggest ni sir na ilagay siya kase dito makakagawa ka ng controls.
▪ Remember that the risk are anticipated. These are anticipation on what might happen wrong.
▪ Kapag na recognize mo yung risk, ‘yung assumption mo is gagawa ka ng control. Yung control mo yun
yung recommended action.

Risk/s Recommended Action

Competitors to OTC drugs
Fraud / scam suppliers and customers
Pandemic
Budget creep
Courier - misplaced or defects
Medical supplies with defects
Economic downturn - fire, flood, or other natural
disasters
Bankruptcy
Baka hindi ka kumita (competition) Conduct market analysis, repackage the services
depending on the market analysis.

So at least may control ka kung paano mo hindi ma

e’encounter ‘yung risk na ‘yun. Develop a marketing plan
parang ganito.
III. Operational Mechanisms: Project Financing and Ways Forward

A. Financing
• Goal is from inception to establishment.
• Ang ending nito is dapat ma-cost niyo siya until establishment.
• Huwag maglagay ng cost na 20,000 lang ah. Kase corporation yan eh
o 3 million
• Kung ang ginamit is pre-implementation, implementation, and post-implementation gagamit ka ng phase.

Work and Financial Plan

Resource Requirements Responsible
Phase/ Theme Activities Timeframe Remarks
Particulars Total Cost Person
Pre- Procurement of
Implementation supplies

Implementation
Post-
implementation
Subtotal
Grand Total

How will this be financed?

• Assume kana rito na may pera ka (huwag mong isipin na as a student nurse) kase prospective ito.
• For example, it will come from cumulative savings, magl’loan ba sa bank, makikipag partner ba sa
developmental partner, makikipag partner ba sa LGU? Mixed ba ‘yan na funding? Write in details on how this
will be financed.
• Again, this is finance. Hindi ito ‘yung kita. Finance meaning is the pondo, paano niyo makukuha ‘yung pondo
for establishment.

B. Sustainability Mechanisms
• What is the business model? Cost of services?
• How can this be sustainable (narrative)?
• Dito lilitaw ‘yung prospective na kita. For example. what is your business model? Ang business model ba ay
socialize or pure profit.
• What is the business model? Cost of services?
• Sa sustainability mechanism pwedeng lumitaw dito ilan ba in terms of number yung prospect clients mo?
For example, we have about (hypothetical) senior citizen 25,000 times ganito times ganyan. You have to
compute it. And may explanation sa may computation.
• Paano pa makukuha ‘yung sustainability mechanism? Aside from the cost, baka kasi may Philhealth package
yan, baka pwede thru Philhealth or baka pwede through insurance. Remember pwedeng makapag
collaborate sa mga insurance para marecover ng maxicare ‘yung program mo edi pwede ka ring kumita
through insurance.

C. Expansion Plan
• How will this be expanded (facility/ies and/or services)? Can be narrative or tabular.
• How will your business expand either in terms of facility? Mag b’branch ka ba or paano mo na-f’foresee na
mag expand?
• Dreaming on how you plan to expand the business
• Kunyare adolescent lang yung service tas ngayon gusto mo na include yung children or young adults.
• Formulated:
o Another branch
o Partnership with schools
o Shopee / Lazada
o Supplier in hospitals
▪ Holy Trinity General Hospital
▪ Arayat Doctor’s Hospital
▪ Dr. Emigdio C. Cruz Sr. Memorial Hospital
▪ Lingap General Hospital Inc.
▪ Dr. Wilfredo G. Cortez Hospital Inc.
o Page (social media)
▪ Facebook
▪ Instagram
▪ Twitter

IV. Conclusions: Implications to Community Health and Community Health Nursing

• Detailed explanation of the effects of the project to community health and community health nursing.
• Examples: convenience, hassle-free, and it could save lives.
• More data is needed here.
• Aside from interpretation ng data, for example, paano siya makakapag contribute sa community health or to
public health?
• There are 2 types. It can be direct or indirect contribution.
o Direct contribution – direct public health problem ang nireresolve nung business niyo. Nareresolve ba
niya ‘yung teenage pregnancy? Not really resolve as in na mawawala. Is it positively impacting a public
health program?
o Indirect contribution – cutting across - pwedeng system – for example, nareresolve ba ‘yung lack of
access to commodities.

V. Reflexivity
• Individual reflections of the members (maximum of 50 words per student) about the C-NCM104
experience.

VI. Visioning
• Individual reflections of the members (maximum of 50 words per student) by answering the question
“what is your hopeful vision of public health in the Philippines?”

Appendices
• This may include the following (whichever is applicable):

▪ Kapag ginawa mo, may drawing board ka, it’s like a blueprint. Hindi kasi pwede yung strike anywhere
kayo.
▪ Ang maganda pag-usapan niyo muna as a group. Ano talaga yung gagawin nating enterprise (package)
natin?
