MATERNAL HEALTH CONCERNS
(Pre - During- Post Pregnancy)

Background Information for Learners

Maternal health is the health of women during pregnancy, childbirth, and

the postpartum period. It embraces the health care dimensions of family
planning, preconception (before attempting to become pregnant), prenatal (before giving
birth), and postnatal (after birth) care in order to ensure a positive and satisfying experience
and reduce maternal illness and death. It revolves around the wellbeing and health of women,
especially when they are pregnant, at the time they conceive an offspring, and during child
raising.

Some women undergo maternal illness, maternal infections and exposure to toxins
and nutritional deficiencies which are risk factors of pregnancy. Others, when conceiving,
experience complications like high blood pressure, gestational diabetes, preeclampsia,
preterm labor or miscarriage and other labor complications. A number of women also develop
post- partum symptoms after giving birth.

The World Health Organization (WHO) has specified that despite the fact that
parenthood has been considered as a satisfying common encounter that is passionate to the
mother, a high level of women experience a great deal of difficulties where they endure
health problems and even lost life.

MATERNAL NUTRITION DURING PREGNANCY

Background Information for Learners

Maternal Nutrition - focuses attention on women as mothers, on their nutritional status

as it relates to the bearing and nurturing of children.

IMPORTANCE OF MATERNAL NUTRITION

Maternal nutrition status affects offspring’s health development significantly during

early embryogenesis, pregnancy, birth and lactation, and subsequently determines health
during growth and even throughout adulthood. On the other end of the human lifetime scale,
the importance of maternal nutrition expands into the time period before conception: Pre-
conception nutritional status not only influences fertility, but also embryogenesis and life-
long health. Predisposition for coronary heart diseases. type-2 diabetes mellitus, and
hypertension can be caused by intrauterine adaptations to fetal malnutrition. Hence,
optimizing nutrition for women during their reproductive period can be expected to have a
great impact on the well being of the next generation.

NEWBORN SCREENING AND THE APGAR SCORING SYSTEM
FOR NEWBORN
Background Information for Learners

After marriage, a couple’s desire is to build a family. Humans are reproduced through sexual
reproduction. In this kind of reproduction, the reproductive cells of man and woman join
together to make a new human cell. A new baby at home is exciting, but it can be scary, too.
Newborns have many needs, like frequent feedings and diaper changes. Babies can have
health issues that are different from older children and adults. Newborn means a child from
the time of complete delivery to 30 days old and New born babies should undergo a newborn
screening.

Newborn screening: Newborn Screening (NBS) is a simple procedure to find out if the baby
has a congenital metabolic disorder that may lead to mental retardation or even death if left
untreated.

Importance of newborn screening: Most babies with metabolic disorders look "normal" at
birth. By doing NBS, metabolic disorders may be detected even before clinical signs and
symptoms are present. And as a result of this, treatment can be given early to prevent
consequences of untreated conditions.

When is screening done? Newborn screening is ideally done on the 48th, to 72nd hour of life.
However, it may also be done after 24 hours from birth.

How is newborn screening done? A few drops of blood are taken from the baby's heel, blotted
on a special absorbent filter card and then sent to Newborn Screening Center (NSC).

Who will collect the sample for newborn screening? The blood sample for NBS may be
collected by any of the following: physician, nurse, medical technologist, or trained midwife.

Where is newborn screening available? Newborn screening is available in hospitals, lying-

ins, rural health unit, health centers, and some private clinics. If babies are delivered at home,
babies may be brought to the nearest institution offering newborn screening.

When are newborn screening results available?

Results can be claimed from the health facility where NBS was availed. Normal NBS results
are available by 7 - 14 working days from the time samples are received at the NSC. Positive
NBS results are relayed to the parents immediately by the health facility. Please ensure that
the address and phone number provided to the health facility are correct. A NEGATIVE
SCREEN MEANS THAT THE NBS RESULT IS NORMAL. A positive screen means that
the newborn must be brought back to his/her health practitioner for further testing.
What should be done when a baby is tested a positive NBS result? Babies with positive
results must be referred at once to a specialist for confirmatory testing and further
management. Should there be no specialist in
the area; the NBS secretariat office will assist its attending physician.

What are the disorders tested for newborn screening?

The disorders tested for newborn screening are:
1. Congenital Hypothyroidism (CH)
2. Congenital Adrenal Hyperplasia (CAH)
3. Galactosemia (GAL)
4. Phenylketonuria (PKU)
5. Glucose-6-Phosphate-Dehydrogenase Deficiency (G6PD Def.)

The behavioral development of a healthy baby:

0 -1 month suckles and smiles
2-3 months vocalizes and controls head
4-5 months controls hand and rolls over
6-7 months sits briefly and crawls
8-9 months grasps and pulls up
10 – 11 months walks with support and stands alone

To assess a newborn baby, you need to know the APGAR scoring system.

APGAR scoring system - is a quick test performed on a baby at 1 and 5 minutes after birth.
The 1-minute score determines how well the baby tolerated the birthing process. The 5-
minute score tells the health care provider how well the baby is doing outside the mother's
womb. In rare cases, the test will be done 10 minutes after birth

Activity 3. Kathryn’s Dilemma

Read the situation carefully. Reflect and answer the focus questions.

Kathryn is a 14-year-old girl who grew up in the province. When she was in grade eight, her
aunt took her to Pasay to be a house helper in the morning and to study at night in the nearby
public school. While studying, she fell in love with one of her classmates who is older than
her. They got married and lived in the squatter area of Baclaran. She got pregnant but because
her husband’s salary is below minimum wage she did not go to any clinic or hospital for
prenatal check-up. She saved money for the coming of the baby. She gave birth in a public
lying-in clinic to an underweight child with several health complications. Alisa and her
husband got so scared about their child’s condition.

Focus questions:
1. What could have prevented the poor health condition of the child?
2. What government agencies could have helped Alisa during her pregnancy?
3. What insights and realization did you gain from the story?
Guide Question:

Explain the importance of new born screening and knowing the APGAR scoring system.

Importance of Prenatal and Postnatal Care

Background Information for Learners:

Prenatal Care is a health care given to a pregnant woman, usually this starts at least
three months before the beginning of conceiving. This includes teaching the mother some
healthy habits such as quitting smoking and drinking alcohol, taking folic acid supplements
daily, talking to your doctor about your medical conditions, dietary supplements and any over
the counter drugs that you take and avoiding all contact with toxic substances and chemicals
at home or work that could be harmful. A visit to an OB-Gynae is very helpful because it can
determine the health condition of the pregnant mother and at the same time the condition of
the baby on her womb. A pregnant mother needs to visit her doctor in the following schedules
every month within the first six months, every two weeks in the seventh and eighth month
and every week in the ninth month. This can help the doctor to determine if there are any
complications during the pregnancy period and at the same time the doctor can prepare the
mother to give a normal delivery. Prenatal care also includes care for the mother by taking
routines of test and screening such as blood test to check for anemia, HIV, and blood type,
monitors blood pressure, measuring weight gain and also monitoring the baby’s growth, heart
rate and also baby’s position and noting changes in the baby. Babies who were born without
prenatal care can have triple chances being born at risk and five times more likely to die than
those whose mothers go for prenatal care.

While prenatal care focuses on the pregnancy period there is another health care given to a
mother after giving birth it is the postnatal care.
Postnatal/Postpartum Care a health care that last for six to eight weeks after delivery or
giving birth beginning right after the baby was born. This period deals with the changes
physically and emotionally that happened during pregnancy period, this postpartum care
involves getting proper rest, nutrition and vaginal care. Getting enough rest is very crucial for
the mother who just gave birth because she needs to rebuild her strength so that she will not
be getting too tired as a new mother. Here some of the things that a mother needs to do in
getting enough rest: sleep when the baby sleeps, keep the bed near the baby’s crib, and allow
someone else to feed the baby with a bottle while the mother sleeps. Nutrition is also one of
the needed postpartum care for the new mother because of the body changes that happened
during pregnancy and labor, but the changes that happened during that period is still essential
for the mother’s condition in giving breast feeding. The experts recommend that breast
feeding mothers eat when they feel hungry, here are some tips for a good diet for a mother
during the postpartum care: avoid high – fat snacks, focus on eating low - fat foods that
balance protein, carbohydrates, fruits and vegetables and lastly drink plenty of fluids. Vaginal
care is one of the essential part of the mothers postpartum care because they may experience
vaginal soreness if they had tear during delivery, urination problems like pain or frequent
urge to urinate, discharge including small blood clots and contraction during the first few
days after delivery. Postpartum care is a treatment that will take care of the problems
occurred after delivery, it is also recommended that there will be no sexual intercourse within
six to eight weeks after delivery.

Prenatal and Postnatal care very important during and after pregnancy because it
assures that the mother is away from any risk that can affect her and the child even after
giving birth the mother needs to stay healthy for the newly born baby.

Importance of Responsible Parenthood

Background Information for Learners

Do you know that…
Parenthood is the state of being a parent. Traditionally, it means being a father or
mother of the biological child. When adoption is involved, the parents are the legal father
and mother of the child.
Responsible parenthood is the will and ability to respond to the needs and
aspirations of the family.
Responsible parenting is defined as the series of decisions couples make to ensure
the best possible life for the family and for the community to which the family belongs. It
is the ability of the parents to raise children in the Filipino way and to satisfy the social,
economic, and religious responsibilities of a family. Parenting also includes the
inculcation of values and instilling of discipline.
Responsible parenthood includes the process of deciding how to choose our
partners, when to form families, when to have the first child, child spacing, how many
children to have in all, etc. It includes all the decisions we take to ensure the best possible
lives for ourselves and our children.
There are several points when thinking of parenthood:
1. Happiness and stability of the marriage.
2. Physical and emotional readiness of both partners for parenthood.
3. Financial stability.

Responsible Parenthood – Effects of Family Size on Health
Background Information for Learners
The Philippines’ population is rapidly increasing and as such, is one of the most
populous country in the world. Much has been said about the impact of a growing population
in the country. These impact can be seen likely in the access to education, health care, food
distribution and opportunity to work. The accessibility has not been fair to all, most especially
on the marginalized group of the society which has more than the planned number of children
or commonly, has extended families. . The partiality is an indication of not having an equal
chance to partake on the resources of the country. Thus, leading to problems that affect not
only the economy of a country but most of all, the health of the people, physically and
mentally. Since the health of the people affects the health of the nation, it is therefore
mandatory that people should be conscious on how to address the effect of population
explosion.
The growing population of a community is based on the number of people in a family.
It is already mentioned that the size of the family matters. A family is considered small if it
consists of 3 to 4 members and big if it consists of more than 5 family members. Family
members depends on who is in the family. Knowing our culture, that is, we value
significantly our family, we tend to extend our family members. It is for this reason that we
have different family structures.
Below are the different family structures.
1. Nuclear families have parents and one child.
2. Couples can be families without having children.
3. Single parent families have one parent and one child.
4. Adoptive families include one or more children who were born into other families.
5. Blended families are made when two families combine (ex. two single parents marry).
6. Extended families include members such as grandparents, uncles, aunts and cousins.
The effects of family size on health can be seen on nutritional status, morbidity, risk
behaviors and utilization of health services. Nutritional status refers to the state of the body
with respect to each nutrient and to the overall state of the body weight and condition.
Morbidity is the exit from a state of physical and psychological well - being resulting from
disease illness injury or sickness. Risk behavior refers to specific forms of behavior which
are proven to be associated with increased susceptibility to a specific disease or ill-health. A
few examples are using and abusing drugs, drinking too much alcohol and more. Utilization
of health services refers to the experience of people as to their receipt of health care services
of different types.
As students, it is your responsibility to educate yourselves for you to be aware on the
effects of family size and be conscious of the consequences. This awareness will guide you to
plan carefully your would-be families, which, in return will give them better opportunities
and fair accessibilities in the future. You should always think of giving the best for your
family.

their family.
The ages of their children are nine, eight, seven, six, three and two months old.
Two of them stopped studying to help Susie do the chores and earn a living. The seven and
the six-year-olds could not finish school too because of their financial needs. The youngest

Methods in Modern Family Planning
(Natural and Artificial)

Background Information of Learners:

Modern Family Planning allows the husband and wife to attain their desired
number of children and determine the spacing of pregnancies. It is achieved through the use
of contraceptive methods and the treatment of infertility now a days.

A woman’s ability to choose if and when to become pregnant has a direct impact
on her health and well-being. Family planning allows spacing of pregnancies and can delay
pregnancies in young women at risk of health problems and death from early childbearing. It
prevents unintended pregnancies, including those of older women who face increased risks
related to pregnancy.
There are two methods in Modern Family Planning, the Natural Method and the
Artificial Method.
Natural Method or also known as the Traditional Methods are the following
examples, the calendar method or rhythm method, women monitor their pattern of menstrual
cycle over 6 months, subtracts 18 from shortest cycle length (estimated last fertile day).
Another example is the withdrawal, man withdraws his penis from his partner’s vagina
keeping semen away from her external genitalia. And for the breastfeeding mother’s they
have this Lactational Amenorrhea Method or LAM.

Artificial Method are also known as the Modern Method with the use of
contraceptive methods, these are the pills, implants, injectable, contraceptive vaginal ring
(CVR), intra uterine device (IUD), condom, vasectomy, and tubal ligation.

Providers can help women, and couples think about their preferences and their
situations and choose the method that best suits them. Many women seeking family planning
services already know which method they want, and counsellors should provide this method
along with accurate and complete information. It is the provider’s job to ensure that a client
makes an informed voluntary choice with their family planning methods.
