 C. SYNOPSIS:

a. That stated time, date and place of activity, personnel of this unit under
direct supervision of PCPT AL-SHAMIR L HAMIN, OIC, PCpl Annie S. Balleta, Asst.
WCPD/FJGAD PNCO of Lavezares MPS conducted information dissemination thru short
dialogue regarding RA 9262 to the residents of the aforesaid barangay. Activity ended same
date with positive result.

b. That stated time, date and place of activity, personnel of this unit under direct
supervision of PCPT AL-SHAMIR L HAMIN, OIC, PCpl Annie S. Balleta, Asst.
WCPD/FJGAD PNCO of Lavezares MPS conducted information dissemination thru short
dialogue regarding RA 8353 to the residents of the aforesaid barangay.

c. That stated time, date and place of activity, personnel of this unit under direct
supervision of PCPT AL-SHAMIR L HAMIN, OIC, PCpl Emily T. Igbuhay, Asst.
WCPD/FJGAD PNCO of Lavezares MPS conducted pulong-pulong, information
dissemination thru film showing and short dialogue regarding RA 9262 and RA 8353 to the
residents of the aforesaid barangay.

d. That stated time, date and place of activity, personnel of this unit under direct
supervision of PCPT AL-SHAMIR L HAMIN, OIC, PCpl Annie S. Balleta, Asst.
WCPD/FJGAD PNCO of Lavezares MPS conducted information dissemination thru short
dialogue regarding RA 11313 to the residents of the aforesaid barangay.

e. That stated time, date and place of activity, personnel of this unit under
direct supervision of PCPT AL-SHAMIR L HAMIN, OIC, PCpl Emily T. Igbuhay, Asst.
WCPD/FJGAD PNCO of Lavezares MPS conducted pulong-pulong, information
dissemination thru film showing and short dialogue regarding RA 9262 and RA 11313 to the
residents of the aforesaid barangay.

f. That stated time, date and place of activity, personnel of this unit under
direct supervision of PCPT AL-SHAMIR L HAMIN, OIC, PCpl Annie S. Balleta, Asst.
WCPD/FJGAD PNCO of Lavezares MPS conducted distribution of flyers to the residents of
the aforesaid barangay.

g. Activity ended with positive result
