SURVEY QUESTIONNAIRE IN PRACTICAL RESEARCH 2 (Group 3 – Salinas’

Group)

The questionnaire would be divided in three (3) sections: the first part would ask for

some of the personal information of the respondents such as their names, age, and

email address, and the name of the company that they work for as well as their position,

in which both would be optional.

The second part would contain the questions that pertain to assessing what

leadership style is being practiced by the leader(s) of the respondent and would be

answered by stating if the respondent strongly disagrees, disagrees, neither agree nor

disagree, agrees, and strongly agrees. Some of the questions were adapted from Ismail

et al.’s (2010) survey questionnaire. The said questionnaire would contain the following

The third part would contain questions that pertain to employee attrition. Similar to

the second part of the questionnaire, the questions in this part would be answered by

stating if the respondent strongly disagrees, disagrees, neither agree nor disagree,

agrees, and strongly agrees. The last part would contain the following questions:
