AFFIDAVIT OF COMPLAINT

I, GEORGE A. PUGA, Filipino, 20 years old, single, a resident of Purok 6 Dimakita St.
Barangay Labo, Tacloban City, after having been duly sworn in accordance with law, hereby
depose and state:

1. That on January 26, 2022 at around 5:30 in the afternoon while I was walking
heading home at Dimakita St. corner Liwanag St., I met Koki A. Maching and Enel S.
Lutang, a resident of Purok 5 Barangay Labo, Tacloban City;

2. That without warning they suddenly attacked me with a baseball bat until I fell down
and keep on beating me while saying “ Ang sama ng tingin mo a, matapang ka b!!?”

3. That they just stop beating me and run away when other people passing by came to
help me.

4. That due to the said incident, I suffered wounds in my head and a broken rib due to
severe beating, and hospitalized for 7 days and advised to complete rest for 3 weeks
or more until my rib will be completely healed.

5. That I am executing this complaint-affidavit in order to file a case of Serious Physical

Injuries against KOKI A. MACHING and ENEL S. LUTANG.

AFFIANT FURTHER SAYETH NAUGHT.
