AFFIDAVIT OF GUARDIANSHIP

ELTON O. SALANDO, of legal age, widow, Filipino, and resident of SNT

Agatha Dita Sta. Rosa, Laguna after having been sworn to in accordance with law, do
hereby depose and say that:

1. I am the biological father of the following children:

a. ELTON JOHN I. SALANDO, 7 years old.

2. I have been the sole guardian of above child ever since his mother,
SHIELA JEAN I. SALANDO died on February 1, 2022;

3. I am not an imbecile, insane, a vagrant or a vicious person or a habitual

drunkard or a habitual criminal, and have not abandoned, neglected or
refused to support said minor or caused him to commit offenses against
the law;

4. As guardian and father of ELTON JOHN I SALANDO, I hereby declare

that I am competent to represent him in all transactions pertaining to his
rights and interest over the property located at Block 18 Lot 3 Phase 1
Southridge Villas Brgy. San Lucas, Lipa City, Batangas left by his mother,
and to sign on his behalf the Deed of Extrajudicial Settlement of Estate or
other related documents; and

5. I am executing this affidavit to attest to the truth of all the foregoing

statements and for whatever legal purpose it may serve.
