The birth of the child, being a vital event, should be registered at the Office of the City Civil Registrar
within a thirty (30) day reglamentary period from the time of birth.

If the child’s parents are not married but would like the child to bear the father’s surname, an Affidavit
to Use the Surname of the Father (AUSF) will also be registered upon registration of the Certificate of
Live Birth (COLB) pursuant to and subject to the limitations of the Revised Implementing Rules and
Regulations of R.A. No. 9255.

2) REGISTRATION OF FOUNDLING
A foundling is a deserted or abandoned infant / child whose relatives are unknown, or a child committed
in an orphanage or similar institution and whose facts of birth and parentage are unknown. The Civil
Registry Law provides that the registration of foundling in the city / municipality where the child was
found shall be made by the finder / charitable institution within thirty (30) days from the date of finding /
commitment of the child.

3) REGISTRATION OF DEATH
It shall be the responsibility of the physician who last attended the deceased or the administrator of
the hospital or clinic where the person died to prepare / accomplish the Certificate of Death and certify
as to the cause of death. The death certificate shall, within forty eight (48) hours after death, then be
forwarded, to the City Health Officer for evaluation and signature prior to registration with the Office of
the Civil Registrar.

On the other hand, it shall be the responsibility of the spouse or any nearest relative who has
knowledge of the death to report the same to the City Health Officer within forty eight (48) hours if the
deceased died at home or any place other than hospital / health facility.

Registration of death includes registration of fetal death or the death prior to the complete expulsion of
a product of conception, irrespective of the period of pregnancy.

4) ISSUANCE OF MARRIAGE LICENSE
Where a marriage license is required, each of the contracting parties shall file separate sworn
application for such license with the proper local civil registrar of the place where either or both of the
contracting parties reside. The local civil registrar concerned enters all applications for marriage
license in registry books filed strictly in the order of receipt. When the license is issued, the same
shall be valid in any part of the Philippines for a period of 120 days from the date of issuance, and
shall be deemed automatically cancelled after the expiration date even if the contracting parties have
not made use of the license.

5) REGISTRATION OF MARRIAGE
For marriage being solemnized in the City of Antipolo, the solemnizing officer or his/her duly
authorized representative or the contracting parties shall register the Certificate of Marriage with the
Office of the City Civil Registrar within 15 days after the wedding rites. For marriage of exceptional
character, the same shall be registered 30 days after the date of solemnization pursuant to Article 34

7) REGISTRATION OF LEGAL INSTRUMENTS


Legal instrument concerning the civil status of a person should be registered also in the Local Civil
Registry. Legal instruments consist of the following:

 Affidavit of Acknowledgement / Admission of Paternity – a public document executed by the


biological father establishing paternal relationship with the child.
 Legitimation – is a remedy by means of which those in fact were not born in wedlock and
should, therefore, be considered illegitimate, are, by fiction, considered legitimate, it being
supposed that they were born when their parents were already validly married.
 Affidavit to Use the Surname of the Father (AUSF) pursuant to Republic Act No. 9255.

10) FILING OF PETITIONS FOR CORRECTION OF CLERICAL
ERROR OR CHANGE OF FIRST NAME OR NICKNAME
Republic Act No. 9048 authorizes the city or municipal civil registrar or the consul general to correct a
clerical or typographical error in an entry and/or change of first name or nickname in the civil registry
without need of a judicial order. An administrative remedy in nature, it is a departure from the usual
judicial process in correcting clerical errors of changing an entry in civil registry documents.

On the other hand, R.A. No. 10172 authorized further the local civil registrars to include the
processing of petitions for correction of entries that involves change of sex and the date and month of
birth. In this case, petitioners may file Migrant Petitions to correct documents registered in other city /
municipality except for correction of entry that involve change of sex.

: Owner of the document to be corrected


: Persons of legal age who have direct and personal interest (i.e. spouse, children, parents,
siblings, and grandparents, guardian or any other person duly authorized by law or by the
owner of the document).

FOR CORRECTION OF CLERICAL ERROR


: PSA copy of Birth Certificate
: At least any two (2) of the following supporting documents that will be the basis of the
correction of clerical / typographical error:
 Birth Certificate of parents / siblings
 Marriage Certificate
 Baptismal Certificate
 School Record
 Voter’s ID / Certification / Registration Stub
 Government-issued IDs
For petition involving change of first name, correction of entries involving sex and date and
month of birth, the following documents will also be required:
 Clearances from Court, Police and NBI
 Publication
 Medical Record
 Certificate of Employment

Note: Depending on the distinctiveness of each case and availability of documents


according to the petitioner, the City Civil Registrar may have to ask client to present
other supporting documents not in the list to support the petition.

If document to be corrected is registered in Antipolo:


: ₱1,000.00 – Correction of Entry/ies
: ₱3,000.00 – Change of First Name
: ₱3,000.00 – Correction under R.A. 10172 (correction of entry involving sex and
date and month of birth)
If document to be corrected is registered in other city/municipality, petitioner will have to
send the filing fee enumerated above to the concerned civil registry and in addition, pay
for the following migrant fee:
: ₱500.00 – Correction of Entry/ies
: ₱1,000.00 – Change of First Name
