Reference No.G-RO/LMC/High Percentage/15-16 dated 06.11.2015 followed by
the Office Order No. S&S-3(Pt.II)/MON/2014-15 dated 19.11.2015 issued by the
Area Manager, FCI, Siliguri on the above subject, it is requested that the following
documents may kindly be kept ready/supplied to the Investigating Team at the
earliest for conducting the said Investigation. The Team is scheduled to reach in
the first week of December,2015.

INVESTIGATION REPORT ON HIGH PERCENTAGE OF TRANSIT LOSS IN
RESPECT OF WHEAT CONSIGNMENT EX-NARWANA, ROHTAK VIDE
R/R NO. 262000263 DATED 24.9.2015 AND UNLOADED AT NCB
RAILWAY SIDING UNDER FCI, DO (COOCH-BEHAR) ON 30.09.2915

In compliance with the Office Order of RO(WB), FCI, Kolkata issued under
Ref.No. G-RO/LMC/High Percentage/15-16 dated 06.11.2015 and subsequently Office
Order No. S&S-3(Pt.II)/MON/2014-15 dated 19.11.2015 issued by the Area
Manager,FCI, Siliguri, the Regional Investigation Team consisting of the undersigned
Members conducted the investigation towards occurring of high percentage of Transit
Loss to the tune of 0.78% in respect of Wheat Consignment Ex-Narwana,Rohtak (Spl.
No. 1014,Dated 24/09/15 and R/R No. 262000263 dated 24.9.2015) unloaded at NCB
Railway Siding under DO, Cooch-Behar on 30.09.2015. Upon due examination and
scrutiny of the available supplied relevant documents and registers, the team hereby
submit its report as under :-

Other Observations

1. Loading Procedures & required formalities were not properly observed at


Consignor’s end.
2. Unloading was supervised by ICCS as well as Officers/Officials from DO,
Cooch-Behar. ( Copy of Office Order Enclosed )
3. Loss reported to Consignor on 01-10-2015 through e-mail within time
limit.
4. Wagons SI condition duly certified by the Rly. Authority ( Copy Enclosed )
5. No team was deputed by the Consignor for joint inspection.
6. M/C range recorded in the ICCS Report of Consignor. ( Copy Enclosed )
7. ICCS Report,Loading Statement and R/R copy supplied by the Consignor.
8. Consignor Claim lodged in time vide No.Movt/C.Claim/FCI/COB/2015-16
dated.05-10-2015. ( Copy Enclosed)
9. All the stocks of the said consignment have already been liquidated and

12 Investigation towards such high %age of TL was conducted by the Area

Irregularities and Lapses on the part of Despatching End.


1. Though dunnage was provided on the wagon floors, Crevices were not plugged
with gunny strips, 18” gap not kept near the flap doors and Bags were loaded
touching the Doors .
2. Texture of Gunny was old & poor. Wagons were not cleaned before loading.
3. As per Loading Statement ,Net weight of 1161 bags is less than 1144 bags and
again Net Weight of 1150 Bags is more than the Net Weight of 1163Bags which
indicates that the bags were of Non standard in weight character and paper
weighment instead of Recording actual weight by the despatching end . Had
proper weighment been taken at the despatching end such variation would not
have been noticed. ( Copy Enclosed )
4. Despite reporting of such high percentage of TL to the tune of 0.78%, no team
from Consignor end reported for joint inspection.
5. No correspondence repudiating such high percentage of TL received from the
consignor’s end.

Irregularities and Lapses on the part of Receiving End.


1. Wagon wise CTL not recorded.

Recommendation of the Regional Investigating Team

In view of the above facts & figures, the undersigned Regional Investigation
Team is of the opinion that several major lapses/violation of instructions on the
part of Despatching end at the time of loading have been observed causing
occurrence of such high percentage of TL to the Tune of 0.78% .

INVESTIGATION REPORT ON HIGH PERCENTAGE OF STORAGE LOSS


SUFFERED IN RICE DURING THE MONTH OF SEPTEMBER, 2015 AT FSD
BUNIADPUR (WEST DINAJPUR)

In compliance with the Office Order of RO(WB), FCI, Kolkata issued under
Ref.No. G-RO/LMC/High Percentage/15-16 dated 06.11.2015 and subsequently
Office Order No. S&S-3(Pt.II)/MON/2014-15 dated 19.11.2015 issued by the Area
Manager,FCI, Siliguri, the Regional Investigation Team consisting of the
undersigned Members conducted the investigation towards occurring of high
percentage of Storage Loss to the tune of 0.38% in respect of Rice Consignment
during the month of September, 2015 at FSD Buniadpur under DO, West Dinajpur.
Upon due examination and scrutiny of the available supplied relevant documents
and registers, the team hereby submit its report as under :

As per existing instructions, the Storage Loss in Rice would be determined


considering the following points :-

1. Full allowance for moisture loss between 15 and 14%.


2. 0.7% SL for every 1% moisture loss below 14% (proportionately)
3. 0.2% SL for 365 days storage period on proportion basis.

Accordingly, on the basis of the above directions/instructions, necessary


workings and assessment of unjustified storage loss has been undertaken and
attached as Annexure-I. The summary of unjustified storage losses and value
thereof are as under :

The value may be revised/modified on receipt of instructions from higher authority


or other relevant rates if applicable in this respect.

However, considering the other reasons highlighted by the Depot/Shed


Incharge such as Avian Pest infestation through the broken upper verandah of Shed
No. , rodent troubles as well as operational losses etc., the above Storage
Losses to the tune of % for the month of September, 2015 appear to be
justified.
