You are safety investigation authority chief.

Initial notification

You have got, information from Latvia ANS provider (LGS), that in the area
of responsibility at November 30 at 21:00 UTC 2 aircraft disappeared from
radar.

Currently known information:

1. Aircraft TU154, registration Russian Federation, RA, Departed from S.


Petersburg to London, Operator „Universal airlines” from Bahrein.

2. Boeing B757, registration Iceland, departed from Riga to Oslo, Operator


company „My country airline” from Latvia.

Occurred over North Sea, at International waters, outside territory of any


state.

Your actions:

 Notification

 What state shall conduct investigation?

 Who should participate in investigation;

 First tasks;

 What investigation groups reasonable to create?

 Aircraft wreckage recovery (actions).


SCENARIO No2
On March, 200X Boeing 777, flight No TS961 (registration Canada) departed
from Varadero, Cuba at 02:48 UTC to Quebec, Canada. At initial phase of
flight, problems occurred with aircraft control (03:15 hours) -apparently over
international waters. Aircraft returned back at Varadero and landed at 04:18
UTC.

After landing it was cleared up that aircraft lost rudder, it separated from
vertical stabilizer and probably fell in ocean. (see Picture 1)

SCENARIO No 3

Cumulonimbus cloud

On August, 200Y, operator Easy Jet Boeing 737 (registration Switzerland) at


09:50 UTC departed from Geneva, Switzerland to Luton UK. After takeoff,
the crew noticed CB and turned to west. Aircraft got into strong hail storm
about 6 sec. over territory of France. There had not radar warning. The crew
declared „mayday” and aircraft returned back to Geneva.

Aircraft had following damages – front edge of wings, stabilizer, engine


intake, cockpit window and radome of antenna. (see Pictures)
