    Time Start: 8:00 am End Time: 10:30 am

    A. Call to order

    Ma’am Arianne: “The meeting will come to order. Good morning everyone. We are
    gathered here today to talk about the Limited Face-to-Face implementation for Senior High
    Students. To begin with may we call in Ms. Chona Mae S. Esteva ( Grade 11 Adviser) for
    the prayer.”
    Prayer: Amen

    B. Roll Call
    Secretary: “As I call roll, will each member please stand up to be recognized. “ Roll call is
    completed. All visitors should be introduced to the group at this time”.
    Welcome Address: Teresita Ngitngit (PTA President)
    C. Approval of the Minutes from the Last PTA Meeting
    -The secretary will read the previous minutes.
    D. Agenda/ Open issues
    Mr. Jonathan Canonigo (Teacher’s Association President) discussed the following:
     Approval for the Division level for Limited Face-to-face
     Presenting of the Parent’s Consent Form.
     Presenting the vaccination status of all Teachers and Students.
     Guidelines and Procedures for the simulation of Limited face to face for Senior High
    students.
