1. For period of December 2021, the number of controlled flies is (104).
There are (10) flies from result after treatment and (94) flies from
flycacther. The number of flies found this month has increase to previous
month (102). This month, flycacther was installed in the vong, le burger,
canteen and loading areas to minimize flies. The flies from result after
treatment found in the in the trash and baiting in the canteen basement 3

2. For period of December 2021, the number of controlled cockroaches is

(6). The number of cockroach findings this month has increase to the
previous month (3).

3. For period of December 2021 the number of controlled ants is (4).The

number of ants findings this month same as to previous month (2).

4. For period of December 2021, the number of controlled mosquitoes is

(451). There are (397) mosquitoes from mosclean and (45) mosquitoes
from result after treatment. The number of mosquito findings this month
has increased compared to the previous month (393). The treatment were
coldfogging in the lobby, vong, le burger, artisan bar, locker, office, and
prayer room. Spraying treatment is done in the basement area, garden
outdoor, gutter, and sumpit. Hot Fogging treatment is done in the basement
and outside area. Mosquitoes are indicated to be sourced from puddle in
the gutter area basement 1, 2, 3, and a sloping ground water area.

5. For period of December 2021, the number of controlled rodent is (13).

There are (8) rodent trapped in the gluetrap and (5) rodent trapped in lie
trap. The number of rodent findings this month has increased compares to
previous month (5)

Action Plan For Flying Insect Treatment

  Hot fogging treatment in the outside area is maximized again especially in basement
area 3rd, 2nd and 1st and garden area
 Cold fogging treatment in the lobby area, vong, and Le Burger is directed up to reach
the top area.
 Larvaciding treatment in the gutter basement 1,2,3, sumpit area, omnia area, and
chiller room area is done every day
 Chemical rotation from deltamethrin to cypermethrin for spraying and coldfogging

Action Plan for Rodent Control

 Routine doing rat poison treatment in outside area every 2 days

 Routine monitoring life trap in the vong, le burger kitchen, garden outdoor and
basement area every day.
 Routine wash traps and change the bait every time we check.

Recommendation

 Maintain the cleanliness of the steaward area and the trash bin area of the canteen
 Ensure that there are no puddle water in the basement 1,2,3.
