JOINT AFFIDAVIT
WE, EDNA VICTOR and LEONORA GALLANO, both of legal ages, Filipinos, and
residents of Victoria, Tarlac, after having been duly sworn to in accordance with
law do hereby depose and state:

1. That we are the acquaintances of REXIE PADER LEONZON;

2. That based on our personal knowledge, REXIE PADER LEONZON was born
on November 19, 1980 from the lawful union of REYNALDO LEONZON
and ELMA PADER;

3. That the Birth Certificate of REXIE PADER LEONZON was not registered
due to the circumstance at that time and the negligence of the person
who was tasked to do so;

4. That we are executing this Affidavit to attest the foregoing statements

and to and for all legal intents it may duly serve.
