JOINT AFFIDAVIT

WE, ARMELITA MARIANO, of legal age, Filipino, and a resident of Sto. Cristo,
Nueva Ecija and LIGAYA DAGDAGAN, of legal age, Filipino, and a resident of
Sto. Cristo, Nueva Ecija, after having been duly sworn to in accordance with law
do hereby depose and state:

1. That we are the neighbors and acquaintances of the deceased CANDIDA

DELA CRUZ BERNARDO;

2. That based on our personal knowledge CANDIDA DELA CRUZ BERNARDO

died on February 1, 1966 in Brgy. Quezon, Nueva Ecija due to childbirth;

3. That CANDIDA DELA CRUZ BERNARDO’s remains was laid to rest in public
cemetery of Brgy. Quezon, Nueva Ecija;

4. That the Death Certificate of CANDIDA DELA CRUZ BERNARDO was not
registered due to the circumstance at that time;

5. That we are executing this Affidavit to attest the foregoing statements

and to and for all legal intents it may duly serve.
