IMPLEMENTATION

This is the phase wherein the community care plan is materialized in order to
meet the goals envisioned to solve the community problems assessed. The
implementation phase must be executed in a way that it is measurable, attainable
and realistic as this is expected to carry-out a positive change in the community
health and development.

A community health nurse role is the promotion of health. By manipulating the


existing variables that restricts and impedes the development of the community, a
certain positive outcome is expected through driving-away these restraining forces.
However, the success of the implementation of a community care plan is directly
affected by the community’s recognition of the problem, and the community’s
dedicated participation to change the underlying concern.

Certain strategies like health-teaching are the most common implemented
plans by the community health nurses. Health education and health teaching
confront existing community-health issues through imparting knowledge that will
create self-sufficiency in the community. Health education serves as a compass on
how to deal with the community-health issues, enabling self-mobilization, and
enriching self-efficacy.

NON-COMPLIANCE TO FAMILY PLANNING

THEME: “Philippine Family Planning Program Solusyon Tungo sa Maayos na


Kinabukasan ng Pamilya.”

Program: Health-education

Prior to implementation, the interview with the selected 31 families of Sto.


Niño, Nacoco, Calapan City, Oriental Mindoro yielded a community concern of non-
compliance to family planning resulting into over-population of the families. This
overpopulation led to different consequences like inadequate nutrition of the family

members, low-educational attainment and unemployment rooted from the low-
attained education.

In response, the nursing researchers came-up with the theme, “Philippine


Family Planning Program Solusyon Tungo sa Maayos na Kinabukasan ng Pamilya.”
An hour health education was conducted among the residents of the community to
supply them knowledge about the significance of conforming in family planning
programs in an early time. Aside from this, proper birth spacing of 4-5 years was also
emphasized for orientation of the better opportunities that may set-in for the family.

The nursing students look forward on the positive outcome that this simple
family planning seminar will cause in the long-run. Through the knowledge supplied,
decline of population among the families of Sto. Niño, Nacoco, Calapan City,
Oriental Mindoro is expected.

Problem No. 02

LACK OF SANITATION

THEME: “Sanitation and its’ Importance.”

Program: Health-education

Prior to implementation, the interview with the selected 31 families of


Sto. Niño, Nacoco, Calapan City, Oriental Mindoro yielded a community concern of
lack of sanitation resulting into unsanitary surrounding. This lack of sanitation led to
stagnancy of garbage in different surroundings leading to consequences like foul

smell in the surrounding, breeding places of mosquitoes and high-risk to contract
diseases.

In response, the nursing researchers came-up with the theme, “Sanitation and
its’ Importance.” An hour health education was conducted among the residents of the
community to supply them knowledge about the significance of sanitized surrounding
and its positive effect on the community. The negative effects of non-sanitized
surrounding were also intensified during the lecture.

The nursing students look forward on the positive outcome that this simple
sanitation seminar will cause in the long-run. Through the knowledge supplied, a
consistently neat and sanitized surrounding is expected to occur in the following
days at Sto. Niño, Nacoco, Calapan City, Oriental Mindoro.

Problem No. 03

LACK OF PERSONAL HYGIENE

THEME: “Reinforcing Personal Protection through Maintaining Hygiene


Practices.”

Program: Health-education

Prior to implementation, the interview with the selected 31 families of Sto.


Niño, Nacoco, Calapan City, Oriental Mindoro yielded a community concern of lack

of personal hygiene resulting into poor upbringing and easily contracting diseases.
This lack of personal hygiene led to anti-social behaviours and poor presentation of
self among the community members.

In response, the nursing researchers came-up with the theme, “Reinforcing


Personal Protection through Maintaining Hygiene Practices.” An hour health
education was conducted among the residents of the community to supply them
knowledge about the significance of personal hygiene and its positive effect on the
community. Side lectures on how personal hygiene will boost one’s immunity was
also discussed.

The nursing students look forward on the positive outcome that this simple
personal hygiene seminar will cause in the long-run. Through the knowledge
supplied, a positive outlook is expected to be seen among the residents of Sto.
Niño, Nacoco, Calapan City, Oriental Mindoro.

Problem No. 04

UNEMPLOYMENT

THEME: “Alternative Livelihoods to Combat Unemployment.”

Program: Health-education

Prior to implementation, the interview with the selected 31 families of Sto.


Niño, Nacoco, Calapan City, Oriental Mindoro yielded a community concern of
unemployment resulting into not meeting the family needs, inadequate nutrition, and

other necessities. Unemployment is the root of poverty as it is directly related to low-
educational attainment; likewise it can be viewed bilaterally.

In response, the nursing researchers came-up with the theme, “Alternative


Livelihoods to Combat Unemployment.” An hour seminar on different alternative
livelihoods was conducted among the residents of the community. Among the
alternative livelihoods were longganisa making, embutido making, and tocino
making. Learning different alternative livelihoods is important as this does not only fill
the gap between unemployment, but also encourage the Filipinos to be
entrepreneurs; being their own boss.

The nursing students look forward on the positive outcome that this simple
alternative livelihoods seminar will cause in the long-run. Through the supplied
knowledge, the community of Sto. Niño, Nacoco, Calapan City, Oriental Mindoro is
expected to be more self-sufficient.

Problem No. 05

LOW EDUCATION ATTAINMENT

THEME: “Ways to Reduce Low Educational Attainment.”

Program: Health-education

Prior to implementation, the interview with the selected 31 families of Sto.


Niño, Nacoco, Calapan City, Oriental Mindoro yielded a community concern of low-
educational attainment resulting into higher risk for unemployment and gradually

transforming into poverty. Unemployment is the root of poverty as it is directly related
to low-educational attainment; likewise it can be viewed bilaterally.

In response, the nursing researchers came-up with the theme, “Ways to


Reduce Low Educational Attainment.” An hour seminar on different governmental
and non-governmental that provides scholarships was relayed among the residents
of the community. Among the agencies that can be applied for is CHED, DOH,
DepEd, other local government units, and even respective barangays. Low
educational attainment only happens if the person in need does not find away.
Despite the poverty and all odds that may block ones way, the government offers
different scholarships that can help young aspiring Filipinos to finish their studies.
The key to combat low-educational attainment is the realization that one can achieve
anything as long as he or she trust himself or herself and work on it.

The nursing students look forward on the positive outcome that this simple
ways to reduce low-educational attainment seminar will cause in the long-run.
Through the supplied knowledge, the community of Sto. Niño, Nacoco, Calapan City,
Oriental Mindoro is expected to have more aspiring degree holders and educated
residents through various scholarships and other means.

EVALUATION

Known as the last phase in the nursing process, the effectiveness of the
conducted interventions is evaluated in this part. The nursing students appraises the
responses made in the community and how the progress is yielding positive impact
in the present time, while continuity of monitoring is uphold in the long-run; to know
how it developed significantly in the community. Evaluating the interventions will
manifest the result whether the aspired objectives and goals were met in the end.

Through the health-education executed, the nursing students are looking


forward for positive results that will be evident in the latter part of the year, as well as
the residents of Sto. Niño, Nacoco, Calapan City, Oriental Mindoro will be more
participative in the following family planning programs of the LGUs.

increase of water-bill is also a contributing factor enlisted by the residents.

Through the health-education executed, the nursing students expect that

bath as this is a way to protect themselves in easily acquiring diseases.

increase of water-bill is also a contributing factor enlisted by the residents, as well as
they do not go outside always thus not having a reason to brush their teeth often.

Through the health-education executed, the nursing students expect that


more residents of Brgy. Sto. Niño, Nacoco, Calapan City, Oriental Mindoro will brush
their teeth as this will prevent the development of cavities as well as it will boost their
self-confidence.

their nails often as this will prevent the risk of acquiring worms in the body and other

community problems we have assessed during our stay which are as follow: non-
compliance to family planning programs, lack of sanitation, lack of personal hygiene,
