MOCK SCENARIO

Actual Situation: On June 19th, 2004 at 1000 hrs local time (Time Zone: GMT + 012 hrs) in position
45 41 00 N / 151 30 00 E, ship on voyage from Tacoma, USA towards Kaohsiung, Taiwan laden with
Bulk Maize. Ship’s course 236°(T), ship’s speed 13.50 knots. Ship southwest of Aleutian Islands.
Current set 100°(T) drift 1.5 knots. Wind direction southwesterly, force 4. Swell direction south
westerly, height 1 to 0.5 meters. Sky overcast, visibility moderate, barometer 1009 hpa, air
temperature 11°C and seawater temperature 8°C.

Simulated Situation: Man fallen overboard from the fore castle deck. Sighted by the Duty Officer
and emergency alarm sounded.

LOG OF EVENTS

19-Jun-04
1000 hrs Third Officer sighted man falling overboard from the fore castle deck on the
starboard side.

General alarm sounded.

Engine room informed.

Master on the wheelhouse.

Engine room asked to put engines to stand by immediately. Man overboard signal
sounded.

Wheel hard over to the starboard side.

1002 hrs Master decided to execute Williamson turn.

Crew mustered in the general muster station. Head count taken. Man fallen overboard
identified.

Emergency stations called.

1010 hrs Engine on stand by rpm.

Extra look outs posted in the wheelhouse.

Lifebuoys made stand by for throwing on sighting the person in the water. Crew
standby by the railings to throw the lifebuoy and also keeping a lookout.

All engine room personnel asked to report to the Chief Engineer in the Engine
Control Room.

All other hands dispersed for their duties.

Man overboard marker set off from the wheelhouse to the position of the incident
tracked.

GPS set on man overboard mode.

Position, wind speed, direction, time, current set and drift established and noted
immediately. Sea temperature noted.

All dangers in the vicinity established. All vessels identified on the AIS.

Not under command signals displayed. NUC lights also switched on.

Emergency lifeboat party reported rescue craft in readiness. Master informed to leave
boat station in readiness and assist the other squads as required.

1013 hrs Both steering motor on. Vessel on hand steering.

All ships in the vicinity alerted of man overboard on VHF and MF/HF DSC.

Initial contact made with the company vide OS-0801-PC.

Completed Williamson turn and now proceeding on reciprocal course towards the
casualty.

1016 hrs Both radars switched on different ranges.

Ship’s position checked. Also re checked weather forecast, wind, current, direction
and force.

1017 hrs Additional lookouts posted from the engine room all over the ship.

1019 hrs Chief Officer informed that pilot ladders were ready and rigged on the lee side. One
pilot ladder stand by on the other side but not rigged.

1025 hrs Safety nets, heaving line, Suez boat davits, man ropes, line throwing apparatus,
stretchers, first aid kit, rigged boat ropes, guest wraps along the boat hull, and hot
water kept ready in bath tub in the hospital. Make shift bath tub made with 200 litre
oil drum.

RPM reduced engines on dead slow ahead. Still making way and on reciprocal
course.

1030 hrs Auxiliary engine no.1 on stand by mode, started and put on load.

1035 hrs All positions established on the chart.

Man overboard marker sighted on the port bow. Vessel proceeded towards the man
overboard marker.

1036 hrs Man sighted waving his hands.

Lowered rescue boat immediately. Position of the man plotted on the radar. Hand
held VHF put to use.

Constant direction provided by the bridge team to the rescue boat to make a correct
approach. Both man and rescue boat established on the radar with bearing and
distances.

1045 hrs Rescue boat reaches man, hauls inside the rescue boat and proceeds towards the
vessel. Man reported alive but in coma.

1110 hrs Boat reaches the vessel. Rescue strops deployed. Boat hoisted. Secured. Man lowered
out of boat. Taken to hospital for medical treatment.

1115 hrs Radio medical assistance received, doctored to the casualty. Casualty improving and
progressing towards normalcy.

No pollution / fire observed.

1125 hrs An urgent management meeting held in the wheelhouse. Possibilities of approaching
a port of refuge were discussed for landing the casualty ashore.

1135 hrs Concluded management meeting. On seeing the improvement in the casualty the idea
was dropped and the vessel proceed towards the original destination. Advised the
agents regarding the casualty and immediate arrangement for ambulance on arrival.

1136hrs Resumed normal operation.

1136 hrs Dead Slow Ahead. NUC shapes and lights removed.

Survival crafts secured.

1145 hrs Vessel gathered speed. Put on auto steering. Confirmed both steering motors switched
on.

Safety message cancelled on VHF and MF/HF DSC.

Owners / Charterers informed through telex.

All emergency parties called off and called for debriefing meeting in the ship’s office.

1200 hrs Crew proceeded for lunch.

MASTER’S REVIEW

The drill was reviewed and was found to be carried out satisfactorily. Some points were high
lightened to the ship’s staff. They are:

All additional aspects were discussed in of rough weather was associated with low temperatures.

It was also highlighted that during such incidents due to panic it may be possible that personnel may
injure themselves. It is always advisable to keep your cool in such situations to avoid any casualties.

All precautions, which could have been observed were discussed by the Master and made sure that all
Officers and Crew understood the importance of it. The situation in real life could have been very
critical.

Other possibilities such as the same emergency in night time that is in hors of darkness would have
been a totally different scenario. Measures and action in such case was discussed in detail.

The drill was carried out satisfactorily with an attempt to make it as realistic as possible. All officers
and crew were requested to go through the company’s procedure S-0907 to be well aware of the
actions to be taken during such an incident.

In association to the above drill the Master discussed the importance of the International Aeronautical
Maritime Search and Rescue Volume I, Volume II and Volume III. He also stressed the duty officers
whenever time permits should read through all these publications for knowledge and instant use
during emergencies.

The Master finally told all the staff regarding the importance of reporting these types of situation to
the flag state and other parties. In our case it is the Hong Kong Marine Department who are to be
notified through their designated forms. They later become notices and are circulated to the all the
other vessels and flag states for the enhancement of safety and security in the world of maritime
industry.
