Directions: This monitoring tool shall be used by the District Heads and School Heads in the monitoring of teachers using
a scale of 1 to 5, where 5 is the highest and 1 is the lowest, rate the performance of the teacher based in your factual
observation and reflect the MOVs that support the rating given.

I. Preparation Activities
1. Has a copy of schedule of Monitoring of
Teachers
2. Has approved documents prepared and given to
the teacher in his/her monitoring of learners.
3. Has monitoring tool in the monitoring of
learners crafted by the school head.
4. Has reflected his/her whereabouts in the
Logbook.
5. Has a clearance from the local IATF that the
place to be visited is safe and secured from
COVID 19
6. Has the necessary PPE as required by the IATF
II. During Monitoring of Learners
1. Has checked the Portfolio of Modules and
Activity Sheets, Worksheets and others.
2. Has presented to the learners and
parents/guardian.
3. Has conducted pleasantries and discussion of
the needs of the results of the
activities/formative exercises in the modules.
4. Has given an in-depth discussion or RRE
Activities to the Learner.
5. Has elicited issues and concerns related to the
lesson focus for the week.
6. Has conducted informal and formative
assessment activities which are contextualized
and localized.
III. Post Monitoring of Learners
1. Has submitted a Travel Report and other
documents as proofs of his/her travel.
2. Has conducted an Action Plan for the
preparation for the next monitoring/visitation of
learners.
3. Has prepared a Differentiated Instructional
Activities for the learners to be monitored.
4. Has crafted additional RRE Materials for
interventional activities.
5. Has submitted the materials crafted for approval
of the School Head.
