I. Diagnosis: Patient is diagnosed with uncontrolled blood pressure. Husband is
diagnosed with chronic alcoholic problems while Son is engaged with smoking and
drinking problems


FAMILY DESCRIPTION
1.) Type of Family- Nuclear Family consisting of Mother, Father, Daughter, and Son.
2.) Family Life Cycle- Launching adult children.
3.) Family Psychodynamics – They are a close-knit family, the daughter and son are both
adults who help with the family in different aspects such as financial and decision
making. The Family has seldom miscommunication. The mother works as a government
employee and the father works as a farmer. Their eldest son just finished college and
their daughter is currently in 3rd year college. They are all open and vocal about their
personal concerns in the family.
4.) Family Assessment- They are all healthy and sought medical assistance when they get
sick. The family members are all healthy although the mother has history of
hypertension. The father, mother and daughter have normal BMI while the son is
borderline overweight. The family has no regular exercise routine except for the
daughter that went to the gym every weekend to exercise. The family have complete
immunization including Covid-19 vaccine given from their local barangay health office.
The family tries to maintain a healthy balanced diet.

FAMILY NUTRITION

A. FAMILY HYGIENE: Family has good personal hygeine including taking a bath everyday, wears
clean clothes everyday, perform proper handwashing before and after eating, proper nail
and teeth hygeine, covers mouth and nose when sneezing, they clean and disinfect objects
and surfaces they often use such as desks and doorknob
B. EATING HABITS. Family eats their meals regularly,performing desirable behaviors during
meals, having breakfast, lunch, and dinner frequently with family members at the dining
table although Charelle often skips breakfast because she has classes early in the morning.
Also they consumed healthy foods with more frequency, including protein foods, dairy
products, grains, vegetables and fruits. Unhealthy eating behaviors such as eating fatty
foods, sweets, soft drinks, salty foods, processed or instant foods, and eating snacks
at night are moderately controlled.

C. STORAGE FACILITIES The family uses gas stove in cooking. Mr. Charlie and Mrs. Mary Ann
are the ones who prepare the food. They cook inside their house.The food that they usually
eat is vegetables, chicken, fish, and pork. When it comes to storing their food, they have
proper food storage because they have refrigerator. In terms of cooking facilities the family
is equip with pots, “sandoks”, utensils and knives.
