(Orientation Phase)
Name of Student Nurse: Joanne M. Perez and Maria Victoria Valencia
Address:
Mr. Kenneth has a major depressive disorder. He is 34 years old and not married. His background
as it follows as stated by Mr. Kenneth, he is an architect who has a lot of workloads to do. As time goes
by, many workloads in his plate that cause his anxiety at first then it turns into depression. He also
mentioned that he tried to reach out for his family, colleagues and friends about his anxiety, but it gets
worse because his bosses are filing his more workload to do and by that time, he cannot bear it anymore
that is why he attempts to suicide. He also mentioned that his attempt suicidal is four times. The patient
was cooperative and friendly throughout the interview, he is well- groomed, attentive and very responsive
for our question and answer. Through of the whole of interview, the patient remained calm and kind.
DESCRIPTION OF ENVIRONMENT:
Dr. Lourdes Pelagio is caring for the patient at Angel Care Custodial Services. The facility
provides a comfortable and clean environment for the patients. The patient is in a large room with other
patients, which may cause other patients to be distracted from their own tasks.
OBJECTIVES:
The orientation phase's goals are to build trust between the patient and the student nurses. It's
vital to build rapport with the client at this phase so that they feel comfortable interacting with the student
nurses. In addition, the student nurse must maintain privacy and confidentiality. At this point, the patient
provides certain basic information. The comfort of the patient must be guaranteed, and the client should
not feel obligated to answer any inquiries.
Process Recording (Orientation Phase)
EVALUATION:
Throughout the virtual interview, the patient was particularly attentive and pleasant. The patient
responds in great detail and tells us everything we need to know. He was easy to work with and
approachable. There are no noticeable behavioral changes. During most of the interview, the patient is
focused and maintains eye contact.
(TERMINATION PHASE)

Mr. Kenneth is more comfortable and relaxed during our termination phase. He communicated
well and share about his day without asking. Patient is neat and well-groomed.
DESCRIPTION OF ENVIRONMENT:
Mr. Kenneth’s environment that day was very busy they were arranging for their upcoming
event which is the anniversary of the facility. Despite their busy schedule, Mr. Kenneth manage his own
to make time for us to conduct for our interview.

We had a pleasant discussion. Even though it was a virtual interview, we formed a rapport.
He's more at ease when answering queries. He was able to recall events from the past. The patient is
relaxed and confident in his responses.
